Project Coordinator

Flemingdon Health Centre (FHC) is a registered charity and an incorporated not-for-profit Community Health Centre (CHC) located in Toronto. FHC provides inter-disciplinary primary health care, health promotion, disease prevention, and community development programs and services. We offer a variety of programs and services to people living within our catchment area (O’Connor-Steeles-Victoria Park-Bayview).

FHC successfully received agrant from the Toronto Urban Health Fund (TUHF) to support an initiative working with Roma families in Flemingdon and Thorncliffe Park. This is a three year grant that will use a community development approach to enhancing skills and resources around healthy sexuality specifically for the Roma communities.

As part of the Health Services Department, the Project Coordinator will be responsible for the efficient and effective delivery of the Resiliency in Roma Communities Project.

Employment Terms: One (1) Full-time, 35 hours/week, position. Some evenings and weekends are required. 3 year contract.

Salary: $39,391-$47,215 plus benefits

Reporting to: Director, Health Services and/or designate

Qualifications, Skills and Proficiencies:

  • Experience working with the Roma communities in a community based environment
  • Knowledge of and demonstrated proficiency in the practice of health promotion and community development
  • Demonstrated experience in outreach
  • Experience planning, implementing and evaluating programs
  • Experience with intake, orientation, and navigation of new clients
  • Ability to work independently and collaboratively in an inter-disciplinary team environment
  • Knowledge of community resources
  • Written and spoken Slovak and/or Romani and/or Czech and/or Hungarian
  • Knowledge of catchment areas is considered an asset

Responsibilities:

  • Plan, implement, monitor and evaluate theResiliency in Roma Communities Project together with project team members
  • Plan and facilitate groups for parents, youth and families
  • Plan and facilitate meetings including planning, evaluation and advisory group meetings
  • Support an advisory group to involve members of the community in the project
  • Support Roma clients, including navigation and liaising with primary care providers
  • Act as a resource to the community and FHC staff by establishing links with external community groups, agencies and organizations
  • Collaborate with stakeholders in the community to identify and respond to needs
  • Produce health education materials
  • Write reports, maintains files, anddevelop documentation/data bases necessary for evaluation
  • Work with students, volunteers and/or community members
  • Participate in internal and external committees
  • Other duties as assigned
