2016 JCJC AWARDS PROGRAM

RESIDENTIAL PROGRAM OF THE YEAR

A residential program is defined as any public or private sector program serving alleged and/or adjudicated delinquent youth in a residential setting. This includes foster homes, group homes, residential treatment programs, general child care facilities, shelter care facilities, detention centers, independent living programs, and other programs providing out-of-home, overnight care to children.

GENERAL CRITERIA

  1. Nominations must be postmarked no later than May 2, 2016.
  2. Nominations must be signed by the chief juvenile probation officer and judge.
  3. Nominations must be written so the nominee's identity cannot be discerned by reading the narrative. The name and county of the nominee should only be written on the front cover.
  4. Nominations may only be withdrawn by the chief juvenile probation officer or judge who signed the nomination form.
  5. Nominations must be submitted on this nomination form, using only the space provided.
  6. Only one nomination per category per county will be accepted.

PLEASE NOTE: Nominations which do not meet all criteria will be rejected from the judging process.
