Foster Agreement
<Rescue Name> and the undersigned person, hereinafter referred to as the foster, promise to agree and be legally bound by this Agreement under the following terms and conditions:
Foster Care: You will provide care for the animal you agree to take and we agree to place in your home at the address found on the bottom of this Agreement. It is important to remember these animals may be frightened and confused. <Rescue Name> suggests you segregate this animal for a short period until the animal can be evaluated by yourself and a licensed veterinarian. The noted segregation period affords the animal time to become comfortable in its surroundings and the foster time to evaluate the animal. Even with this precaution you are assuming a risk. <Rescue Name> will not be liable for any claims concerning this undertaking.
The foster will provide the following for this animal:
· Clean drinking water and wholesome food appropriate to that particular animal
· A clean and safe environment inside your home
· A clean and safe place for elimination
· Transportation to and from the veterinarian
· Administering medication should that be necessary
· Monitored daily exercise in the form of walks and/or play in a secured yard
o Cats are never to be taken outside unless it is for a veterinary visit, and then only in a crate.
· Socializing and basic training
· Evaluation of that animal to be reported to <Rescue Name> regularly
· A crate or confined place inside the home where the animal can be safely kept if left alone
If the crate is used for housetraining purposes that crate will allow the animal movement without allowing elimination.
A safe place is defined by an area restricting the animal from the main body of the home that keeps them safely confined. A bed, matter for elimination and water will be included in that area.
Adult animals may not be crated for more than 8 hours per day but may be crated over night. Puppies under the age of 6 months may not be crated longer than their age in months. Very young puppies under the age of two months may not be crated for more than two hours without regular breaks.
We require that you notify <Rescue Name> before vacations to inform us of arrangements made for this animal.
Veterinarian Care and Expenses: <Rescue Name> will be responsible for all medical care for the animal placed in your home. You must notify <Rescue Name> of the need before an appointment is made. <Rescue Name> will require a copy of the veterinarian's bill for reimbursement. All <Rescue Name> Board Member phone numbers will be provided to you in case of an emergency.
