BUILDING COMMMISSIONING SERVICES – PREQUALIFICATION

FOR 2006 BOND PROGRAM

This is a non-binding, Request for Qualifications (“RFQ”). Responses to this RFQ will not be considered as firm offers, nor will responses result directly in an award for these services. Responses may or may not be used by the Boulder Valley School District, (the “District”), as an aid in creating a future Request for Proposal.

The District is requesting pre-qualifying information via a response to this RFQ from building commissioning agents.

The intent of this RFQ is to pre-qualify selected commissioning agents to provide building commissioning services during the design and construction of new buildings and/or the renovation of existing buildings which will occur during all phases of the District’s 2006 Bond.

The bond program is expected to be implemented in three phases with bond lettings in 2007, 2009 and 2011. Request for Proposals (RFPs) will be issued periodically throughout the Bond program to secure building commissioning services. Firms that fail to provide a response to this RFQ will not be eligible to perform building commissioning services.

While the information gathered through this request will be helpful to the District at this preliminary stage, it will not be a basis for final firm selection. Contract awards will be based upon District issuance of separate proposals as commissioning services are required on projects, review of responses, and Board of Education discussion/approval, if required.

For purposes of this RFQ, “building commissioning” is defined as a systematic process of ensuring that all building systems, (including, but not limited to, building envelope, structural, HVAC, mechanical control, electrical and life/safety systems) perform properly, without interruption, according to the contract documents, the design intent and the District’s operational needs during the pre-design, design, construction and post-warranty phases of the project. On behalf of the District, the “commissioning agent,” who will be under direct contract with the District, would be responsible for planning, organizing, and facilitating the building commissioning process. The commissioning agent will integrate and perform the functions of design peer review, equipment start-up, control system calibration, testing, adjusting and balancing, documented functional testing and verification, equipment documentation and facility staff training, including correctly complied operating and maintenance manuals.

I. Introduction and Overview

This RFQ contains all necessary information to submit a response to the District. Please review it carefully, and if specific questions arise, forward them to the e-mail address listed herein, or fax with an e-mail to confirm receipt. No telephone calls, please.

The Boulder Valley School District is committed to providing a challenging educational experience for all its students. The district serves a diverse community that demands educational excellence, including the communities of Boulder, Louisville, Lafayette, Superior, Broomfield, Nederland, Ward, Jamestown and Gold Hill. The district covers approximately 500 square miles and serves an estimated 140,000 residents, with an enrollment of over 27,600. The University of Colorado and many nationally known companies are located within its boundaries. Many year-round recreational activities are available in the Boulder area. The district is accredited by the Colorado Department of Education.

On November 7, 2006, the voters of the Boulder Valley School District approved Ballot Measure 3A authorizing the issuance of $296.8 million in bonds for district facility improvements set forth in the Educational Facilities Master Plan that was approved by the Board of Education on June 13, 2006.

The original planning for the Bond issue called for the issuance of debt in these increments: $120 million in 2007, $100 million in 2009 and $76.8 million in 2012. These amounts were based upon a preliminary six-year, three-phase project list.

The District envisions pre-qualifying commissioning agents and submitting proposals to select pre-qualified firms, on an as-needed basis, for new building construction and for the renovation of existing buildings.

C. Vendor Response Format

One (1) original of the vendor response, signed by a person duly authorized by the firm to sign on behalf of the firm, plus five (5) copies must be received in the District’s Bond Planning, Engineering and Construction Department, by the deadline of Friday, January 11, 2008, at 2:00 P.M. (Mountain). Email responses will not be accepted.

III.  RFQ Response Submittal Conditions

a.  Submitting commissioning agents shall provide all information requested in the RFQ.

b.  A single, complete RFQ response must be submitted for each commissioning agent.

c.  The RFQ must be signed by a duly authorized official of the commissioning agent submitting the RFQ. If the commissioning agent is organized as a partnership, association or joint venture, a duly authorized official of each firm shall sign the RFQ. Reference Attachment A.

d.  Only one RFQ response will be accepted from any person, firm or corporation.

e.  RFQ responses shall be prepared in a comprehensive manner as to content, but no necessity exists for expensive binders or promotional material.

f.  All costs, including travel, printing, copying and any other related expenses incurred in the preparation of this proposal and/or future participation in any proposal process shall be borne solely by the commissioning agent.

The District desires and encourages that responses be submitted on recycled paper, printed on both sides. While the appearance of responses and professional presentation is important, the use of non-recyclable or non-recycled glossy paper is discouraged. Brevity and specificity are expected.

Commissioning agents shall submit the following responses in order within their response to this RFQ.

Responses must be in sufficient detail to provide for a thorough evaluation and assessment of a commissioning agent’s qualifications to provide professional services on District projects.

IV. Required Responses

A.  Information presented in the RFQ response should conform to the following format:

A.1 A one-page letter of introduction is not required but may be included in the RFQ response.

A.2 Information submitted must be arranged in the same order in which it is requested in items B.1 – B.19. The RFQ response shall be page numbered.

A.3 Submittals are to use “GBC” spiral bindings only.

B.  The RFQ response shall include the following components:

B.1 Cover Page

The cover page will show the following:

a.  Firm’s name.

b.  Firm’s address.

c.  Firm’s “Statement of Qualifications” and the date of the submittal.

d.  The name of the person(s) authorized to make representation for the firm and that person’s address and telephone number.

B.2 Table of Contents

Provide a Table of Contents for the information included in the RFQ response, including page numbers.

B.3 Completed Attachment A, Signature Page

B.4 Organization and management information

a.  Complete the “Organization Information” form (Attachment B) that follows this section.

b.  Provide an organization chart indicating the staffing, by position and name. The organization chart is to identify proposed sub-consultants, if any, and their discipline or specialty.

c.  If an association, partnership or joint venture is proposed, indicate the scope of work to be done by each firm and the address of each firm where it will be performed. Indicate where and how quality control design coordination will be performed.

B.5 Evidence of Firms’ Fiscal Stability: Provide an audited financial statement for the last two (2) years and quarterly financial statements for the current fiscal year. Or, in lieu of an audited statement, a financial statement that has been compiled and reviewed by an independent accounting firm may be substituted for a privately held business.

B.6 Insurance: Confirm firm’s ability to obtain the insurance specified in the Contract to the $1,000,000 required limit.

List your insurance carrier and agent complete with address, phone numbers, and the names of contact persons.

a. Identify all current insurance coverage, with limits and exclusions identified.

b. How long have you been with this insurance carrier?

c. Have you ever been denied a policy?

d. Have any claims been filed against any policy?

B.7 Resumes of key personnel. Resumes are to be one, single-faced page per person.

a.  Include the resumes of key personnel that will be assigned to the project. Include projects which demonstrate the depth of professional experience on building commissioning. Identify any K-12 school commissioning projects. Indicate the position and responsibility the individual held on each building commissioning project and the dates (month and year) the individual was involved with the project.

b.  Provide a statement that your firm is willing to sign a “key personnel clause”, ensuring that the proposed staff will be assigned for the project duration.

B.8 Examples of relevant commissioning projects:

a.  Provide a list of all building commissioning projects completed in the last three (3) years. For each project, identify the following information:

·  Project Name/Description

·  Owner/Client, and their telephone number

·  Type of Facility

·  Renovation, Addition, or New Construction

·  Construction Value

·  Date of Completion

·  Type of building system(s) commissioned

·  LEED Certification, if any

b.  Use the “Examples of Relevant Projects” form (Attachment C) to provide up to four (4) examples of the most relevant projects your firm has completed in the last three years.

B.9 Describe experience in the following categories:

§  The testing, design, specification or installation of commercial building mechanical, electrical or control systems.

§  Direct responsibility for project management of commercial construction or installation projects.

§  Writing functional performance test plans and assembling a complete commissioning plan.

§  Working with project management teams.

§  Metering and monitoring.

§  Testing and balancing (TAB).

§  Functional performance testing (FPT) of equipment and systems.

§  Installing and/or troubleshooting direct digital controls and energy management systems.

§  Planning and delivering Operating and Maintenance (O & M) training.

B.10 Current building commissioning work under contract

Identify all building commissioning work currently under contract:

Provide the following information for each project:

·  Project name

·  Owner/Client, and their telephone number

·  Type of Facility

·  Renovation, Addition, or New Construction

·  Start Date – Detail specifically the commissioning service(s) provided

·  Finish Date

·  Current Schedule Status of Project

B.11 Experience of proposed subconsultants

If your firm lacks sufficient experience or expertise to address certain building equipment and/or systems, and you are proposing to team with other sub-consultants to address all systems being commissioned, then provide a list of proposed sub-consultants and experience of their key staff with building commissioning projects: For major sub-consultants provide the following:

·  Resumes of all key subconsultant personnel, indicating building commissioning projects of similar nature on which individuals have performed and position and responsibility of individuals on those projects. Subconsultant resumes are to be one, single-faced page per person.

·  Provide a list of the most relevant building commissioning projects during the past three (3) years on which the firm has worked with the proposed subconsultants.

B.12 Provide documentation on any commissioning certifications received by your firm from a nationally recognized organization (e.g., ACG “certified commissioning authority” or CxA).

B.13 Building commissioning experience with renovation and addition projects in occupied buildings.

Describe the firm’s building commissioning experience with projects that involved additions and renovations to existing buildings while they were occupied.

B.15 Experience with Green Build Technologies and LEED Certification.

Describe the firm’s (and proposed sub-consultants) experience with green built and/or LEED Certified projects. Include both K-12 and non K-12 commissioning projects as well as new construction and renovation of existing buildings.

B.16 Termination from or failure to complete a project

Has the firm ever been terminated from a project before completion, or has the team failed to complete a contract awarded to it? If so, please provide a detailed explanation.

B.17 Special or unique experiences and/or qualifications

Identify any special or unique experiences and/or qualifications of the team.

B.18 Litigation/Claims

Provide documentation regarding any litigation or claims made against the firm within the past five (5) years. Include information as to whether the firm is named as plaintiff or defendant, and the nature and status of the claim. If the litigation has been resolved, describe the resolution.

B.19 Conflict of Interest Disclosure

List and describe any conflicts of interest that the firm, individual staff members and/or any proposed sub-contractors may have.

V. General Information

1 Vendors responding to this RFQ must submit their response in the overall format as outlined in this RFQ.

2. An original and five (5) copies (6 sets total) of the response are to be submitted no later than 2:00 PM (Mountain), Friday, January 11, 2008, to:

The original response must be clearly marked. The original response must be dated and signed in ink by a person who is legally authorized to bind the vendor to the response, with that person's name and title clearly identified.
