Request for Proposal: Print Optimization Project

Fayette County Memorial Hospital (FCMH), a county owned hospital that serves Fayette and the surrounding counties and operates under ORC § 339 and follows the competitive bidding process outlined under ORC § 307.87. FCMH is currently designated as a Critical Access Hospital (CAH), is accredited by The Joint Commission (TJC), and provides 24/7 Emergency Department services to the area.

FCMH is releasing this request for proposal (RFP) for a fleet of MFP or printer output devices critical to hospital function & patient care and for the critical back office hospital function due to changing EMR system to athenahealth with go live on February 19, 2019.

Applications identified as critical in considering print solutions are:

Pre-printed stock – Business Office
UB
1500 Health Insurance Claim Form
Wrist Bands – laser Band Advanced
Label Stock – Integra I-V30
Lab Labels
Prescription Paper; printers with locked trays
Print Shop opportunity

The response to this RFP should include the following information at a minimum:

Name of your group including address, the primary contact person, telephone and fax numbers, and email address.
List the make and models of all MFP’s and/or printers along with the number of units required.
Describe any other “value added” services your group may offer.
Provided itemized information on rates for black & white prints and colored prints.
Provide a list of facilities including addresses, telephone numbers, and contact persons as references.
Provide a clear statement of the financial terms of the proposal.
Provide a transition plan and timeline for the initiation of this service if you are the successful provider.
