1. A quotation of proposed supplemental insurance product(s); their description, to include coverage requirements and limitations; product costs, quoted in per month increments to be payroll deducted monthly, and other detailed cost proposal information for each type of supplemental insurance to be offered for employee self-purchase. We will provide an Excel spreadsheet with census information for five employees that can be used to develop rates for comparison purposes. In the development of these comparison rates it should be assumed the employees are non-smokers with no disqualifying attributes.

The supplemental health products we would like to offer to your employees are TermLife, cancer, and accident. The employer pays no part of the employee’s premium and in fact saves money with a section 125 plan.

Group Term to 100 and our Group Term Paid up at 65 life insurance offered by Liberty National is pure insurance protection for your employees and their families. They have guaranteed rates locked in at their present age with no increase in premium and no decrease in coverage as long as the premiums are paid. The coverage can continue after they leave employment and remain in effect till age 100. Our Group Term brochure is included in this packet.

Our cancer product is supplemental. Liberty’s Cancer Endurance Plan provides benefits for the treatment of cancer, including new or experimental treatment. The policy is guaranteed renewable for life as long as premiums are paid on time. Cancer Endurance Plan includes a specified Dread Disease benefit for cystic fibrosis, diphtheria, encephalitis, Lou Gehrig’s disease, meningitis, multiple sclerosis, muscular dystrophy, osteomyelitis, poliomyelitis, rabies, scarlet fever, sickle cell anemia, smallpox, tetanus, tuberculosis, tularemia, and typhoid fever. Payments go directly to the insured, unless they request otherwise, so the funds can be used as they wish. The plan is portable so that if the employee leaves your employ they may take their insurance with them and pay the premiums through bank draft at the same rate. Our Accident Protector Max product is designed to provide coverage for certain losses resulting from a covered accident on and off the job. Policy pays in addition to workers’ compensation. Benefits are paid directly to the policyholder unless otherwise directed in writing. Product is renewable to age 65. Benefits include: Accidental death and dismemberment; initial and daily hospital and intensive care coverage; emergency treatment; coverage for dislocations and fractures; ambulance and transportation; blood and plasma; and waiver of premiums for extended confinement.
