REQUEST FOR PROPOSAL FOR FUNDRAISING CONSULTANT SERVICES

GHFA 2017 CAPITAL CAMPAIGN PLAN

The Georgia Healthy Family Alliance (the 501(c)3 philanthropic arm of the Georgia Academy of Family Physicians) plans to launch a capital campaign in 2017 to raise $3-$3.5 million dollars to expand the nationally recognized Community Health Grant Awards program and the Tar Wars tobacco education and prevention program in Georgia. Toward this goal GHFA has already secured a $1M pledge from the Georgia Academy of Family Physicians.

In support of these efforts, the Georgia Healthy Family AllianceBoard of Trustees seek the assistance of an organization or individual who has a proven track record in developing and executing successful multi-year capital campaigns for non-profit organizations.

Founded in 1987, the mission of the Georgia Healthy Family Alliance is to serve the people of Georgia by enhancing their well-being through educational and outreach programs that promote healthy practices consistent with the principles of family medicine.

The Georgia Healthy Family Alliance is the only charitable organization in Georgia whose objective is to improve access to quality health care through initiatives and programs lead and supported by the care and generosity of family physicians.The Alliance is governed by a Board of Trustees comprised of 11 members including family physicians, corporate representatives and family medicine residents. Statement of Need In January 2016, the Georgia Healthy Family Alliance (GHFA) began its fifth year of the Community Health Grant Awards Program. Since its inception, the Community Health Grant Program has awarded more than $100,000 to support dozens of Georgia Family Physician member sponsored healthcare projects throughout the state that enhance the well-being of our communities. The Community Health Grant Program was also named the 2015 American Academy of Family Physicians Foundation Program of the Year.

In the past 12 months, GHFA has received more than 25 applications for community health projects with resources to fund just nine of the projects. As the Alliance aspires to new levels of community engagement and impact, a new fundraising strategy is needed to continue to assist Georgia Family Physicians helping families in their communities -- whether they face an urgent health crisis, a devastating disease or simply seek to improve their quality of life.

Scope and Guidelines

The purpose of this Request for Proposal is to retain a fundraising consultant who will 1) design a multi-yearcapital campaign plan in order to raise $3-$3.5 Million by 2021; 2) manage the implementation and launch of the capital campaign in January 2017); 3) provide training and guidance tostaff and campaign leadership to ensure the campaign remains a priority for the organization and 4) provide progress reports and modifications to the campaign as needed to maximize results.

The goals of the capital campaign include identifying internal prospects and outside funding opportunities as well as reinvigorating existing donors in order to expand GHFA program offerings over the next five years. Requirements include:

  • Develop capital campaign plan and calendar
  • Develop and supplement prospect research through targeted identification, involvement and stewardship efforts
  • Work closely with volunteer Board members and staff
  • Liaise with prospective, current and past major donors to the Foundation
  • Location is flexible, Southeast based organization preferred
  • 2-3 groups that you have partnered with in the past including contact information

Qualifications Please submit:

  • An overview of your company/skills and any relevant experience;
  • A description of three non-profit projects (include sample of marketing materials, grant proposals, websites, photos of donor parties, etc.)
  • Names and contact information for a minimum of three current references
  • What are your expectations of the staff and Board?
