Part A: Introduction

1  The Expression of Interest (EOI)

Expressions of Interest (EOI) are sought from interested organisations or consortia for the provision of a Health service within the Hume GP Super Clinic (HGPSC). This is an exciting community-based primary care initiative developed under the Australian Government’s GP Super Clinics Program. Dianella Community Health will start to deliver services from these premises in February 2015.

The Hume GP Super Clinic has been developed by Dianella Community Health (DCH) as a new multidisciplinary clinic with a focus on excellence in clinical care and teaching.

The purpose of this document is to provide information to applicants to assist in the preparation and submission of Expressions of Interest that meet the Health service requirements of HGPSC.

2  Indicative timetable

Key dates for this Expression of Interest process are listed below. This timetable serves as a guide to the expected schedule, although DCH reserves the right to vary dates as necessary. Any amendments to this timetable will be notified to interested parties.

Part B: Background

4  Dianella Community Health

Dianella Community Health Ltd (DCH) is a major community health provider in Hume City in the northern fringe of metropolitan Melbourne, which is one of the largest and fastest growing areas in Victoria. DCH operates from six facilities across Meadow Heights, Roxburgh Park, Craigieburn and Broadmeadows.

DCH is a company limited by guarantee governed by an independent Board of Directors and is a registered health promotion charity (HPC). Its clinical, governance and administration systems are fully accredited through QICSA (Quality Improvement & Community Services Accreditation). DCH partners with Medicare Locals, general practitioners, hospitals and three levels of government, as well as local agencies and community members, to achieve an effective universal primary healthcare system.

DCH currently employs over 220 full and part time staff and delivers more than 173,000 services to clients each year. The comprehensive range of clinical and non-clinical services[1] includes restorative and maintenance programs, aged care, Aboriginal and Torres Strait Islander services, child health, youth and family services, early intervention and chronic disease management (including specialised diabetes and cardiac care), dental services, disability services, general health care, population health and preventative services, psychological and counselling services.

5  The GP Super Clinic Program

The development of GP Super Clinics is a key component of the Australian Government’s Health and Hospital Reform Program. GP Super Clinics are expected to support the delivery of integrated, multidisciplinary primary care services and the training and education of the future primary care workforce. Each clinic brings together GPs, practice nurses, public and privately funded allied health professionals and other health care providers to deliver primary health care that is tailored to the needs and priorities of the local community.

Although there is not a prescriptive model for GP Super Clinics, there are a number of core characteristics (objectives) that each funded clinic are expected to demonstrate, including:

§  Well-integrated, multidisciplinary, patient-centred care

§  Responsive to local community needs and priorities, including the needs of Aboriginal and Torres Strait Islander peoples, as well as older Australians in residential aged care facilities and community-based settings

§  Accessible, culturally appropriate and affordable care

§  Support for preventive care

§  Efficient and effective use of information technology

§  A working environment and working conditions that attract and retain the workforce

§  High quality best practice care

§  A viable, sustainable and efficient business model

§  Support for the development of the future primary care workforce

§  Integrated with local programs and initiatives

6  The DIANELLA Super Clinic proposal

6.1  The Super Clinic site

Dianella Community Health has purchased 42 – 48 Coleraine Street, Broadmeadows Victoria, as the site for the Hume GP Super Clinic which is centrally located within the Broadmeadows transport and shopping hub, and bordered at the intersection of Pearcedale Parade. This area is witnessing significant development and expansion.

The clinic building is a two-storey development with total floor space of 1,300 m2 and includes:

§  Integrated clinical treatment and workstation consulting rooms

§  Multipurpose group rooms (treatment and meeting)

§  Pathology

§  Radiology Pharmacy

§  Reception areas, including waiting area

§  Toilet and shower facilities

§  Staff room and kitchen

§  Equipment and consumables storage room

§  IT and office store room

Client car parking is available on site and is adjacent to parking at the Broadmeadows Town and Shopping Centre.

6.2  MBS Room Specifications

The nominated room is located on the level 1 of the building. There are appropriate washroom facilities, reception and waiting areas are available. (access is DDA-compliant)

The nominated room space is approximately [13]m2 and is fully fitted out and will include:

§  1x voice/data + 2x GPO connections

§  Access to a computer

6.3  The Service Model

The service model is designed to address the demographic and health needs profile for the municipality of Hume on Melbourne’s urban outer northern fringe It is envisaged that the Hume GP Super Clinic will grow to include the services outlined below in the model of care.

The following strategies to achieve service integration are flagged in the proposal:

§  A model of intake services that supports state and national reforms in aged care, Home and Community Care and disability together with MBS.

§  Facilitation of care continuity across primary, community and acute services as needed.

§  Functional linkages through shared information, shared guidelines and staff to coordinate care.

§  Structured agreements that reflect a team-based, coordinated and integrated approach to patient care.

6.4  Operational assumptions

The following operational assumptions have been made:

§  The Dianella Super Clinic will operate 8 am to 8 pm from Monday to Friday 9am to 5pm Saturday and 10am to 5pm Sunday. These hours will be implemented by December 2015.

§  After-hours consultations will be looked on favourably and will be progressively introduced into the Super Clinic, depending on demand.

§  Services delivered by GPs are expected to account for 70–75% of patient contacts each year.

§  Services provided by nurses are expected to account for 15% of patient contacts,

§  Services provided by allied health services will account for 10% of patient contacts each year.

6.5  Governance arrangements

As HGPSC is a program within DCH, the DCH Board is ultimately accountable for the services provided by HGPSC.

The following diagram summarises the governance arrangements for HGPSC.

Key features of these arrangements include:

§  The General Manager located at the Dianella is a senior manager within DCH.

§  All private allied health providers will be expected to liaise with the General Manager, Well-being and Coordinated Care

§  GPs, regardless of the arrangements negotiated for their engagement, will be expected to report through the GP Clinical Director.

Note that patients of the Dianella Super Clinic medical practice will be patients of DCH however patients of any private practitioners will belong to the services provider. This should be reflected in individual and shared marketing conducted by both organisations.

Part C: Conditions of Submission

7  Format and content

Submissions are to be structured as set out in Part F: Response Schedule. Applicants may provide supporting documentation as attachments, which may include promotional materials or other documents, provided they directly address selection criteria or other requested information.

9  Closing time

The deadline for lodging submissions is 5:00 pm on 10/04/2015[date] (AEST). Late submissions may be accepted, but this is at the discretion of DCH

10  Ownership of submissions

All submissions become the property of DCH once lodged. DCH reserves the right to disclose the contents of submissions to non-DCH employees who are assisting with the EOI evaluation process. Submissions will not be returned to respondents.

11  Your costs

All expenses and costs incurred in connection with preparing and lodging this EOI will be the responsibility of applicants.

12  Not a contract

This document is a request for expressions of interest, not a contract. Nothing in this document – or in an applicant’s response to it – should be construed to give rise to any contractual obligations or rights.

13  DCH's rights

DCH reserves the right, at any time, to:

§  Alter, amend, vary or end this EOI and the process outlined in this EOI.

§  Suspend or terminate this EOI process or any part of it.

§  Require additional information or clarification from EOI applicants.

§  Enter into further negotiations with any interested party or negotiate with multiple parties.

14  False or misleading claims

If an applicant is found to have made false or misleading claims or statements, or to have obtained improper assistance, DCH may reject that applicant’s submission from any further consideration.

15  Collusive tendering

Applicants – including their officers, employees, agents and advisors – must not engage in any collusive tendering, anti-competitive conduct or any other similar conduct with any other applicant or person in relation to the preparation or lodgement of a submission or the EOI process.

16  Conflict of interest

Applicants must include in their submission details of conflicts of interest or potential conflicts of interest if selected to provide a health services at Dianella Super clinic.


Part D: Specification

DCH is committed to the delivery of high quality, patient-centred, primary health care services to all patients of the Dianella Super clinic and seeks expressions of interest from relevant organisations to provide a Health service. The Health Service applicant should be able to provide a three days/week service with the possibility of extended hours weighted favourably. After hours will be tied to the commencement of GP extended hours service.

16.1  Non-negotiable components of the requirement

The following are the features of the Health service that must be provided:

§  Provision of comprehensive timely electronic reports to referral agents in line with Medicare requirements and standards.

§  Adherence of provider staff to the clinical, infection control and safety protocols in place at the Super Clinic.

§  Provision of services for specialised populations including paediatric clients, people with disabilities, people from culturally and linguistically diverse backgrounds, Aboriginal and Torres Strait Islander people.

§  Provision of culturally sensitive and appropriate client information

§  A commitment to abide by DCH's site policy and procedures including, where appropriate, incorporation of DCH site policy and procedures into guideline documentation.

§  Assist in ensuring HGPSC is an environment attractive to staff and positive to the experience of patients/clients and the local community.

16.2  Negotiable components of the requirement

The DCH Board is prepared to consider a range of options for contractual arrangements with Health service providers and applicants are expected to give consideration to these issues and indicate their proposed approach or preferred arrangements in their application. Points for negotiation include, but are not limited to:

§  The rental that will be paid.

§  Preferred length of the rental term and other lease arrangements.

§  Details of signage. Please note that no dedicated external signage is able to be erected, placed or displayed on the GP Super Clinic building or external grounds of the site.

§  Any additional service or contribution the Speech pathology Service Provider may be able to make to Dianella Super Clinic, including the delivery of extended-hours, participation in teaching programs.

§  Dedicated parking

The Dianella Super clinic is equipped with reception support. There are three options available for Reception assistance which are negotiable:

To support the Health Service Provider within HGPSC, applicants should note that DCH will provide the following:
