2INFORMATION
2.1Section 40 of the National Health Service Wales Act 2006 places a statutory duty on each local authority and local health board to prepare a Health, Social Care and Well-being Strategy for that local authority’s area.
2.2Sections 25-28 of the Children Act 2004 require local authorities and key partners to co-operate across the range of functions to improve the well-being of children, young people and their families. It also places local authorities under a statutory duty to prepare and publish Children and Young People’s Plans.
2.3On the 20th July 2010 the Welsh Assembly Government issued the Children and Young People’s Plan 2011-14 Interim Guidance and the Health, Social Care and Well-being Strategy Guidance for consultation.The guidance sets out the requirements for the next planning cycle and that the plans to be adopted by the 31st March 2011.
2.4The Childcare Act 2006 requires each local authority to undertake a Childcare Sufficiency Assessment.
2.5The Children and Families Measure 2010 requires each local authority to produce a Child Poverty Strategy and the draft planning guidance specifies that this must be included in the Children and Young People’s Plan.
2.6On the 12th August 2010 a decision was made by the Children and Young People and Health, Social Care and Well-being Partnerships that a Joint Plan be produced for Wrexham, this decision was endorsed by Executive Board on the 7thSeptember 2010.
2.7“Our Joint Plan” was subsequently drafted and issued for the statutory twelve week consultation period from the 8thNovember 2010 until the 30th January 2011. The Plan includes both the Child Care Sufficiency Assessment and Wrexham’s Child Poverty Strategy.
2.8A Consultation Timetable, attached at Appendix 1,was implemented to facilitate consultation with citizens, professionals and interested groups. Individual responses were also invited.
2.9Wider consultation with elected Members took place through an All Member Workshop held on the 20thJanuary 2011.
2.10In total approximately 670 people were involved in the consultation.
2.11The final version of “Our Joint Plan”, attached at Appendix 2, has been produced in light of the Consultation responses and comments received from the joint meeting of the Social Affairs Health and Housing Scrutiny Committee and the Children and Young People Scrutiny Committee held on the 16thFebruary 2011.
2.12“Our Joint Plan” is also being presented to BetsiCadwaladr University Health Board on 23rdMarch2011 for approval.
2.13The Council’s Executive Board considered “Our Joint Plan” on 8th March 2011 and resolved that:
i)They endorse Our Joint Plan for adoption by Council
ii)They note that the Council’s ability to fully achieve the Outcomes outlined in “Our Joint Plan” is dependent on the resources made available by all of the Partners.
3RECOMMENDATION
That the Council adopt “Our Joint Plan”, Wrexham’s Children and Young People’s Plan and Health, Social Care and Well-being Strategy 2011-2014 as contained within Appendix 2 of this report.

FOREWORD
We are pleased to present Our Joint Plan which encompasses the Health, Social Care and Well-being Strategy and the Children and Young People’s Plan 2011-14. The purpose of Our Joint Plan is to improve the well-being of children, young people and adults, in Wrexham, through a partnership approach.
This key strategic document has been developed through Wrexham’s Health, Social Care and Well-being Partnership and the Children and Young People’s Framework Partnership, and demonstrates our commitment to both partnership working and public engagement. It contains our three year vision; the outcomes and priority areas that we wish to achieve by 2014 and further strengthens the work we undertook within our 2008-11 plans. In the current financial climate we recognise the importance of ensuring that the highest priority is given to those with the greatest needs in order to achieve our outcomes, whilst ensuring that a focus on prevention is maintained. We will continue to work in partnership with local communities to make the best use of all our resources in order to achieve our vision and demonstrate the difference made to the well-being of the people of Wrexham.

OUR VISION
‘To improve the well-being of people in Wrexham through a partnership approach’
Under the Children Act (2004) the Welsh Assembly Government requires each local authority in Wales to co-operate with partner agencies to develop a plan to improve outcomes for children, young people and their families. The NationalHealth Service Wales Act (2006) places a duty on each local authority and health board to prepare a Health, Social Care and Well-being Strategy for their area. Since both the plan and strategy will run concurrently for three years from April 2011 and together they focus on improving outcomes for all local citizens, the relevant statutory partnerships in Wrexham have agreed that a single plan enhances co-operation, reduces duplication and best meets the needs of the people of Wrexham. A range of key national and local documents have been used to inform the development and content of Our Joint Plan to realise our joint outcomes:
Our outcomes form a jigsaw that together contribute to the well-being of people and show the cross-cutting nature of the work that we do, e.g. enabling people to make healthier choices has a positive impact on all other outcomes
The current challenging financial climate may impact on our ability to fully achieve our outcomes, nevertheless, we are committed to working through partnership to maximise the use of available resources to make our Plan succeed.
SECTION ONE – INTRODUCTION AND CONTEXT
Partnership working-wider context
This Joint Children and Young People’s Plan (CYPP) and Health Social Care and Well-being Strategy (HSCWBS) has been developed by Wrexham’s Children and Young People’s FrameworkPartnership (CYPFWP) and Wrexham’s Health, Social Care and Well-being Partnership (HSCWBP).It provides us with an opportunity to take a life course approach to planning and delivering services. These services aim to improve health and well-being, reduce social exclusion and health inequities and help eradicate child poverty. Our health and well-being is affected by many factors and determinants, such as where we live, our environment, access to services, genetics, income, employment and education levels, behaviours, lifestyle and relationships with our family, friends and the community in which we live. This Plan is designed to take these into account to meet the identified needs of the diverse population of Wrexham.
Our partnerships are made up of organisations (both public and Third Sector) involved with adults, children, young people and their families, bringing together a range of knowledge and experience of working together to improve the well-being of people in Wrexham. These statutory partnerships are part of a wider infrastructure contributing to improving the quality of life for Wrexham citizens. The diagram below gives an overview of the plans and strategies currently operating and reflects how they relate to the Community Strategy.
