Report for Wessex CCGs

This report sets out the surgeries that have completed the steps to become dementia friendly (DF), those in the process of completing this work, and those that are being contacted over the next period.

Background

The Wessex AHSN is working across Hampshire, Dorset, IOW and south Wiltshire to spread this initiative. The timing and detail of the process involved in becoming a DF surgery differs from surgery to surgery, but there are some basic steps that are undertaken. These involve staff training, patient and carer engagement, links to the voluntary sector in the locality, an environmental audit, dementia-focussed care planning, and completion of tools (such as ‘This is Me’) to support the person with dementia and their carer. West Hants CCG has recently encouraged all surgeries in the New Forest to become accredited with the Wessex AHSN and work is underway in New Forest to support these surgeries.
