Repeat Prescriptions Can Be Obtained As Follows

Once a repeat prescription has been agreed and set up by the doctor, you can obtain your regular prescriptions without having to see a doctor each time. On the right hand side of your prescription is a re-order slip detailing the medication you are allowed to have on a repeat basis. You will need to be reviewed by the doctor from time to time. The date of your next review will be shown on the re-order slip – this is often on an annual basis. If you do not attend for review, you will not be issued with a prescription until you have seen the doctor again.

Repeat Prescriptions can be obtained as follows:

1. In Person

Tick the items you need on your re-order slip and hand it in at the Reception Desk Monday to Friday, 9.00 to 5.00. Your prescription will beready to collect 24 hours later so long as this is a working day

2. Post

Tick the items you want on your re-order slip and send it by post with a stamped and self-addressed envelopeto:

This service can be used all through the year. If you do not enclose a SSAE, we cannot return your prescription to you and you must collect it from theSurgery.

3. Re-order your prescription through a Pharmacy

We operate this system with a number of local pharmacies. Leaflets for each of these Pharmacies are available at the Reception Desk. They explain how the arrangement works for that Pharmacy

4. Fax and collect

You can fax us your completed re-order slip on 0121 687 3054 and collect your prescription 24 hours later if the fax is sent in working hours or the second working day for faxes sent out of hours or at weekends.

5. On-Line or Telephone viaPharmacy2U

This company is an NHS approved internet pharmacy which enables you to order your prescriptions securely on line to be delivered to an address of your choice. Please ask for a leaflet or see their website. There is no charge for the mailing but deliveries have to be signed for. They will deliver anywhere in the UK.

6. Email and collect or nominate to collect from one of the above pharmacies

Tthis service is available by obtaining an access PIN at the Reception Desk. See the on-line services section of the website or contact reception.
