Candidates for this assessment:

*Once a student is chosen, e-mail the student’s name and grade to Katherine McLaughlin and Kathy O’Neill for record-keeping purposes.
*Please send home with the student a copy of Dr. Spence’s brochure as well as a copy of “Psychological Services – Outline for Parents”
Required Documentation:
Please ensure the following information is sent via e-mail to Dr. Spence, ideally 2 weeks in advance of the scheduled assessment.
1.  SE 04: Confidential Request for Psycho-Educational Assessments
2.  SE 03: Request for Consent to Provide Psychological Services
3.  Behaviour documentation
4.  Speech and Language assessment
5.  Copy of all JUNE report cards
6.  IEP
7.  Brigance
8.  Audiogram
9.  Vision report
10.  Other professional assessments (i.e., OT, PT, Educational assessments, etc.)
Assessment Process:
·  Psychological Assessments will take place in Ottawa at Dr. Spence’s office.
·  Parents/guardians are asked drive their child to Ottawa. Mileage will not be provided.
·  Families are asked to arrive at 9:30am unless otherwise arranged/scheduled.
·  Students/guardians are asked to bring a lunch.
·  Parents will meet with Dr. Spence at 9:30am regarding student background information and a psychometrist will begin the testing with the student.
·  Testing will take approximately 4 hours (may be longer in some cases).
Feedback Session Process:
·  Dr. Spence is more than willing to speak to the school team via Conference Call regarding results should there be a need.
·  Please e-mail Dr. Spence for a date/time to set up the Conference Call.
Assessment Results:
·  SERT can email Dr. Spence approximately 1 month after assessment is complete to inquire about the completion of the report.
·  Dr. Spence will fax a copy of the completed assessment report to the school (you may need to request this via email) once she has discussed the results with the student’s parents/guardians.
·  If the assessment results contain SEA recommendations (computer hardware, software, etc.), be certain to contact your Coordinator regarding the process for a claim.
Considerations:
·  Does the family have private coverage?
·  Is there another professional service/agency involved that can identify/diagnose and make recommendations?
