THIS LIVE PERFORMANCE AGREEMENT is made and entered into as of (Date of Contract, below), by and between CLIENT, (Purchaser) and REMEDY MUSIC ENTERTAINMENT INC. (Musician).

Please provide a basic timeline of individual events below if applicable: Ceremony, cocktail hour, first dance, cake cutting, pictures and dinner.

ln consideration of the mutual covenants herein contained and, intending to be legally bound hereby, the Purchaser and Musician agree as follows:

1) ENGAGEMENT. The Purchaser hereby engages Musician to render a musical performance and Musician hereby agrees to render such Performance under the terms and conditions specified herein.

2) INDIVIDUALS COMPRISING MUSICIAN. Musician consists of the following: Karin Elizabeth and/or additional musicians if requested and agreed upon by both Purchaser and Musician. Musician's obligation to perform hereunder is subject to the unavailability of Musician as a result of sickness, accidents, acts of God, emergencies and other reasons beyond Musician's control.

5) PAYMENT. (a): ln full consideration for all services rendered by Musician at the Performance, Purchaser agrees to fulfill payment in cash, certified check (payable to Remedy Music Entertainment Inc.), barter or online (Gigmasters) in U.S. funds to Musician in full amount set above (Total Price).

(b) DEPOSIT. Non-refundable.To be paid in full in form and amount specified above on date of contract.

(b) REMAINING BALANCE. To be paid 10 days prior to performance in above specified form to Musician.

6) CANCELLATION. ln the event Purchaser cancels the Performance less than 30 days prior to the scheduled date, Purchaser will pay Musician a sum equal to 50% of the set fee specified in contract. Upon payment of the cancellation fee, Purchaser will have no further liability to Musician hereunder.

7) OUTDOOR EVENTS. Musician and equipment used at above event must be provided with adequate protection (a sturdy tent, etc.) in case of rain or other potentially hazardous weather situations. Performance fee is non-refundable if performance is cancelled due to weather, acts of God or other extenuating circumstances.

8) MISCELLANEOUS. This Performance Agreement set forth the entire agreement between the parties, and may not be amended except in a writing signed by both parties. This Performance Agreement will be governed by and construed in accordance with the laws of the State of Michigan, without regard to the principles of conflicts of law. ln any action or proceeding involving a dispute between the Purchaser and the Musician arising out of this Performance Agreement, the prevailing party will be entitled to receive from the losing party reasonable attorney's fees. Musician and Purchaser have each caused this Performance Agreement to be signed by its duly authorized representative.
