  • Experience of successfully developing and managing collaborative community-based health and wellbeing interventions
  • Implementing business and project plans; successfully achieving targets including managing budgets, producing reports and working to deadlines.
  • Outreach, engagement and working directly with residents to develop and inform service delivery.
  • Supporting, motivating and managing volunteers and/or staff.
  • Implementing monitoring and evaluation systems and reporting to funders and other stakeholders.

  • Proven track record of working successfully with GPs, health providers and community groups to deliver measureable improvements in health outcomes

Job Description

Post: Social Prescribing Co-ordinator

Job Purpose:To develop, deliver and manage the Southmead Development Trust’s Social Prescribing Programme. Working closely with GP practices and other healthcare professionals, the social prescribing programme will aim to address issues that may be causing or exacerbating health problems.The social prescribing service is not in itself a support service – the aim is to work with individuals to identify their needs, then refer or signpost to other organisations, services or community groups as appropriate, some of which may be established as part of this programme.

Salary: £23,000

Hours:37hrs/week

Key tasks and responsibilities

  1. Establish a social prescription programme and referral system across Southmead ward by working with local GPs, health providers and community groups.
  1. Build a ‘menu of community services’ with the aim that GP patients can be signposted into community services and will experience improved health and wellbeing and a reduction in their use of GP practice resources.
  1. Develop (in consultation with the Clinical Commissioning Group Lead) a system for referral and reporting for the programme.
  1. Work with individuals to identify their needs and then refer or signpost to appropriate organisations, groups or services.
  1. Organise sessions such as Healthy Eating and Art on Referral subject to demand.
  1. Build a resilient community self-help health infrastructure by helping to develop and nurture an active volunteers network.
  1. Be involved in developing external funding bids and supporting other income generating activities including developing tender responses to sustain health and well-being initiatives.
  1. Monitor, evaluate and report on project outcomes and delivery of activities.
  1. Work collaboratively with Community Plan partners to deliver appropriate Community plan actions
  1. If funding becomes available extend the project to other areas of deprivation in North Bristol i.e. Lockleaze and Lawrence Weston
  1. Maintain links with the Healthy Living Consortium at an Operational Level
