Course Request Form

This form applies to students who needs help to either seek approval to waive the academic restrictions or register major courses on their behalf. Please read the terms and conditions before you complete the form.

Important Terms and Conditions

  1. The Undergraduate Programme Office (UPO) shall assign any available index to the requested course(s).
  2. This form is not applicable under the following situations:
  • Student fails to register for a single course, as he/she had missed his/her assigned registration period. In this instance, the student must register his/her courses during the first two days of Add/Drop Period.
  • Student wants to change the timetable to accommodate GER-PE/UE, DIP or FYP.
  • Student wants UPO to register GER-PE/UE on his/her behalf.
  1. UPO reserves the right to change some of the classes or even the entire timetable to ensure that students are registered for all their requested courses. Upon completion, UPO would not revert to the original timetable, if the student is not satisfied with the registered index.
  2. Students are strongly advised not to change/drop the allocated index numbers of the requested course(s). Do note that there are very limited vacancies available during the Add/Drop Period.

Offline Registration Form (Academic Warning/Probation)

This form applies to students who have attained Academic Warning/Probation after the release of the examination results. Students would be barred from accessing STARS to register the courses. Please read the terms and conditions before you complete the form.

Important Terms and Conditions

  1. The form must be endorsed by the student’s Academic Mentor before submitting it to the Undergraduate Programme Office(UPO).
  2. Students can only register their normal study load. Overloading is strictly not allowed.
  3. UPO will not register courses for which the student had failed the pre-requisite.
  4. UPO will only register the Core and Major Prescribed Elective Courses for the student’s current year of study. All GER-Prescribed/Unrestricted Electives will be placed on waitlist.
  5. UPO is unable to guarantee whether the student’s choice indexes will be allocated, if the form is submitted after the stipulated deadline.
  6. Students are strongly advised NOT to make changes to their timetable, after the form has been processed.
