Required Activities that earn credit
- $100 due September 1, 2014 Football Mania
Earn $50 toward fundraising obligation
- $100 due November 1, 2014 for Bishops Golf Classic Car Raffle tickets
Earn $75 toward fundraising obligation
- $100 due April 15, 2015 for Our Lady of Lourdes Festival Raffle tickets
Earn $50 toward fundraising obligation
Earn credit as part of a Bingo team, events coordinator, sports team/coach.
Earn extra credit through SCRIP gift cards year round, catalog items in the fall and candy sales in the spring.

PARENT PARTICIPATION PLAN
GRADES JR. K – 8TH
2014-2015
PIP – Parent Participation Plan; 30 hours for the school year or $1,500
PIP hours are not considered fundraising.
- 12 hours working Bingo- 3 evenings
Schedule convenient dates in the school office; sign in and out with cashier at Bingo
- 3 hours working at Our Lady of Lourdes Festival, May 2nd & 3rd, 2015
Sign in and out with Booth Chairperson
- Other Volunteer hours – sign in at event, classroom or in the sign-in book in the office.
Approximately $25 to $30 dollars in donations = 1 hour
Examples of Donations for PIP hours
5 cases of water or soda = 1 hour
3 dozen cupcakes or donuts = 1 hour
NO SHOW FEES
If you sign up to work any event and do not show up, you will be charged a $100 fee.

Athletic Fees:
There is a $45.00 per student fee for Football & Volleyball
Due on October 15, 2014
There is a $45.00 per student fee for Basketball & Soccer
Due on February 15, 2015
There is a $45.00 per student fee for Track
Due on March 15, 2015
Graduation Fee:
The 8th Grade graduation fee is $ 150.00 to cover the costs.
Due on February 15, 2015
Field Trips:
Trips are planned by teachers, cost is determined at that time.
Sacraments:
The Sacraments of Reconciliation and Eucharist are offered for students in and above the 3rd grade who are baptized. The fee is due September 15, 2014
Spirit Shirts:
Available at student store for $10
Yearbook:
Due on September 15, 2014 - $25

AUTHORIZATION FOR USE OF PHOTOGRAPH / IMAGE
I/we hereby authorize the Diocese of San Bernardino and Our Lady of Lourdes School to use photograph/images of (Print name of subject) ______to promote and communicate through a slide presentation, photo display, printed material, posters, internet promotions, electronic multimedia and/or billboard displays.
I agree that this photograph/image shall be free for use in the above-mentioned areas by Our Lady of Lourdes School and the Diocese of San Bernardino without charge or obligation, and further, release and hold harmless the Diocese of San Bernardino and Our Lady of Lourdes School and all their employees, volunteers, affiliated church or diocesan entities or organizations and any of the agencies with whom it is associated, from any liability or obligation in connection with the use of said photograph/image.
It is understood that said photograph/image will not be used for any for-profit commercial purposes without express, written approval.
