"My 4-H Activities" (ages 8-18)
Everyone should turn in this sheet
County Level
1.Everyone receives some type of recognition for turning in this sheet - i.e. seal, sticker, name in program.
2.Divide into age groups - then look over.
3.Should turn in sheets from previous years to show progress.
4.Will not be judged against each other - will be used as a resource (for progress) for other judging.
5.If more space is needed, you may attach another sheet of paper.
"Planning for My Project Adventure" (Ages 8 - 11)
1.Everyone should turn in.
2.Everyone will receive some form of recognition for turning in.
3.Outstanding records will be recognized.

5.Workbook pages from project manuals may be turned in.
"Illinois 4-H Project Plan" (Ages 12 - 14)
1.Everyone should turn in.
2.Everyone will receive some form of recognition for turning in.
3.Outstanding records will be recognized.
4.Should turn in sheets from previous years to show progress.
5.Workbook pages from project manuals may be turned in.
"Illinois Project Goals and Accomplishments" (Ages 15 - 18)
1.Everyone should turn in.
2.Everyone will receive some form of recognition for turning in.
3.Outstanding records will be recognized.

"Illinois 4-H Award Application"
Accept if older 4-H'er turns in voluntarily, or if judges feel older 4-H'er in county has outstanding project records and would have a good chance in state competition.
"Illinois 4-H Livestock Record"
1.To be used for all livestock projects.
- Everyone enrolled in livestock should turn in - may use optional Rabbit or Horse and Pony Record if you prefer for those projects.
3.Everyone will receive some form of recognition for turning in.
4.Outstanding records will be recognized.
5.Should turn in sheets from previous years to show progress.
“Beef Rate-of-Gain Booklet”
1.To be used for 4-H’ers enrolled in Beef Rate-of-Gain project.
2.Everyone enrolled in Beef Rate-of-Gain should turn in.
“Secretary Record Books, Treasurer Record Books and Reporter Scrapbooks” are also to be turned in at this time.
Award Applications available to turn in to Kankakee County
COUNTY AWARDS
1.Anyone 12 years of age or older of current 4-H year is eligible for county awards.
2.4-H’er must submit complete set of records. 4-H'er does not have to be in project area more than one year.
3.County Medal Winners must score 75 points on Awards Application to win award.
4.Candidates may receive a state-provided award medal only once in a specific award category.
5.Continued success in any curriculum area may allow the member to be presented a County Blue Award Certificate.
6.Application Form must be turned in with records at the end of the 4-H year (Date will be given to Leaders and printed in Extension Link).

Mechanics & Technology (Aerospace, Bicycle, Computer, Electricity, Safety – all areas, Small Engines, Tractor, Woodworking, Welding)
Personal Development (Communication, Photography, Visual Arts, Leadership, Recreation, Theater Arts and Reading)
Plants & Soils (Plant and Soil Science, Crops and Soils & Horticulture)
"I DARE YOU" AWARD
1.Nomination Form must be turned in to the Unit Office.
2.Nomination should be from adult who has extensive contact with nominee (4-H leader, project leader, etc.), preferably not a parent.
3.Nominations should be submitted by October 1.
4.Nominees should:
a.Be at least a High School Junior.
b.Have been a 4-H member for at least three years (may be in third year of membership).
5.Nominee should exhibit development in leadership, scholarship, and character not only in 4-H but also church, school, and community. Each of these areas should be included in recommendation.
6.Award may be received only once.
"FRIEND OF 4-H AWARD"
1.Nomination Form must be turned in to the Unit Office.
2.Recommendation should be submitted by October 1.
3.Award is presented to non-4-H individual or group who has contributed significantly to support of 4-H programs.
4.It is suggested that the award is presented only once to an individual or group.
"ALUMNI AWARD"
The Alumni Award provides recognition to 4-H alumni whose contributions to society exemplify effective citizenship, leadership, and public service success in their chosen careers and a sustaining interest in 4-H and similar worthy community endeavors.
1.Nomination Form must be turned in to the Unit Office.
2.Recommendation should be submitted by October 1.
3.Nominee may be any adult who has been previously enrolled in 4-H and who has an outstanding record of accomplishments relating to good citizenship in adult life.
4.Nominees should have achieved success in their chosen profession or occupation.
5.Nominee's record of accomplishments should indicate interest in and support of youth, civic, public, church, or school activities in community and county.
"KEY AWARD"
1.Nomination Form must be turned in to the Unit Office.
2.Recommendation should be submitted by October 1.
3.Nominees should be at least a junior in high school.
4.Proof of nominee's meeting award requirements should be in written form.
5.Nominee must meet Leadership Standards, listed in Clover, for 2 years to receive Key Award.
The Leadership Standards are:
1.Establish plans for member's project and report on project to leader.
2.Attend at least two-thirds of club meetings (or number determined by club).
3.Share project experiences by giving a talk or demonstration or making an exhibit.
4.Serve on club committee or hold a club office.
5.Take part in a community service activity or in a community activity related to member's project.
6.Assist leaders with a special club activity, tour or program.
7.Participate in one or more learning experiences at the county, regional, or state level.
8.Provide leadership for a project, activity, or 4-H group.
9.Help conduct programs at the local level or beyond.
10.Meet the above items for a minimum of two years, and submit county records for the same two year period.
