Hero Information Sheet

1. Talk to your parents or grandparents about heroes. Discuss someone in your family or in your circle of family friends that is a hero to you. If your parent or grandparent is your hero, talk to him/her about why he/she is your hero.

2. Interview your hero. Be sure to ask all of the questions below, but be careful to not get too personal or make him/her feel uncomfortable. You can also interview relatives and friends of your hero to gather information.

3. Gather all of the information on this sheet and bring the sheet to class to start work on your presentation.

4. You have until ________________________________to complete this form. Use personal interviews (be sure to take notes), email, telephone (be sure to take notes), or US Mail.

5. Ask for at least one picture of your hero. It can be a current photo, or one from their past. If they don’t have one, take their picture and develop it. You will need at least three pictures for your presentation (see your rubric), so gather pictures about events or things related to your hero as well.

D. Ask your questions and write your responses. You may end up with a lot of questions and answers. Just remember you are doing a presentation about your hero, so stick to the type of information that will show your classmates why this person is a hero. If you need to, add paper to this form for your answers.
