Directions:

Use this assessment for program planning and for reporting student performance.

  1. For the pretest, mark C (for “Competent”) in the column provided if a student is competent in that task. Competency is achieved when the student can perform the task consistently (100% of the time) in at least one setting with minimal support (no more than two prompts). If the student doesn’t demonstrate this level of competency, leave the column blank. Add up all C’s and place the total in the box provided, labeled “Pretest C’s”.
  2. For the posttest, mark C in the column labeled “Post C’s” is student has achieved competency. If not, use the following scale to show progress:

Adapted from EVALS (TSBVI, 2007). Revised July 2012.

Student Name: Evaluator: Date:

  1. 1=minimal progress
  2. 2=moderate progress
  3. 3=substantial progress
  4. NI= not introduced

For example, if a student has made substantial progress but has not achieved the competency level, you should mark the posttest column with a 3. Use the comments space to give details and explanation of progress. Here’s the trick: Do NOT repeat pretest C’s in the posttest column! Only mark NEW C’s in the posttest column.

  1. Once the posttest has been marked, add all NEW C’s and place in the total in the box provided. Also add up the number of 1’s, 2’s and 3’s and place those totals in the box provided.
  2. Record only the number of C’s in the chart below. Turn this page in to the designated ECC data collector.
