Re: Questionnaire regarding Quantitative Risk Analysis class

Dear Course Participant,

We are sending you a questionnaire regarding the Quantitative Risk Analysis(QRA) class.

QRAis used to address and support a broad range of decisions under uncertainty and risk in business, government and other situations. EpiX Analytics’ classes are designed to find an optimal balance between theimportant methodological basis of QRA and its implementation in many real-world examples and applications.

In order to make the QRA courses even more effective, interesting and relevant, we’re reaching out to all course participants with a questionnaire to better understand your needs and objectives from taking the class, and to ask you to contribute one or more case situations or example Excel models to the class. The use of these cases and example models can truly enhance your learning-experience. While we can’t always guarantee that all contributed examples, cases and models will be used during the courses, we will do our best to accommodate as many as possible.

Please complete this questionnaire and send it back to Ms. Barbara O’Neill () at least 10 days before the start of the class. Also, you can always contact uswith any logistical questions about the course at +1 303 440 8524.

We look forward to welcoming and meeting you!

Sincerely,

Ms. Barbara O’Neill
