Scope

The post holder will co-ordinate the Swindon Home from Hospital Service. This service provides time-limited care and support, both practically and emotionally to individuals following a hospital stay. The service supports people who do not have existing support networks and support is provided for approximately six weeks by local Red Cross volunteers. The scheme is service user focused, helping to build confidence and independence to rebuild and sustain levels of independence.

Overall Purpose of the Post

To provide daily co-ordination and supervision of the Home from Hospital services and volunteers from a hospital setting. To ensure a high standard of service delivery in the provision of the Home from Hospital services.

Main Duties and Responsibilities

  Based predominately within Great Western Hosptial, providing the daily co-ordination of the Home from Hospital service in Swindon.

  To line manage volunteers and a small team of staff.

  Meet with identified service users and conduct an assessment of need with them to establish the level of support required, deliver and coordinate delivery of the service as appropriate.

  To keep in regular contact with potential referrers to the service within the hospital (especially those involved with hospital discharge), visit hospital wards regularly to raise the profile of the service, and attend multi-disciplinary team meetings on some wards where possible, building good working relationships within the hospital.

  Ensure that service promotion is effective, including attending hospital staff meetings and department marketing.

  To respond to and process referrals to the service without delay, keeping accurate written and electronic logs of activity undertaken.

  To recruit volunteers for the services, and implement all BRC policies concerning volunteers.

  To identify individual volunteer training needs, implementing BRC policies concerning training for Home from Hospital volunteers. Ensure that high standards of service specific training are provided for the Home from Hospital volunteers as appropriate.

  To provide support and leadership to Home from Hospital volunteers and keep them motivated.

  To make volunteers delivering the Home from Hospital Service aware of Health and Safety regulations concern their service and their own responsibility for Health and Safety.

  Monitor and review service activity (providing management information and reports as required) and support the evaluation of service quality by various means including collecting service user feedback.

  Administer the Home from Hospital office/work station.

  To participate in fundraising activities where possible i.e. Red Cross Week

  As appropriate, to respond to an emergency call-out by the BRC.

  To undertake any other duties as requested by the Services Manager, Senior Services Manager or Operations Director.

Education/ Training

/   Educated to GCSE level or equivalent**

Experience

/   Knowledge of services to be co-ordinated

  Knowledge of POVA (Protection of Vulnerable Adults) and Child Protection legislation**
  Knowledge of Safeguarding legislation**

  Ability to collate and interpret a range of management information including statistical information and user feedback**
  Ability to identify and assess people’s needs**
  Ability to promote the Home from Hospital service and to raise its profile**
