6. BRIEF RESUME OF THE INTENDED WORK

6.1 INTRODUCTION:

Ayurveda is the science of life is an age old science deals with the both aspect i.e. prevention as well as curative aspect.Panchakarma is considered prime therapy in ayurveda because of its purificative and multifaceated effect on the body,acc to ayurveda vitiated doshas are the causative factor of the all diseases,once purification is done in a particular disease one can cure and prevent recurrence of the diseases.

Difficulty in micturation is known as mootrakricchra, there is pain while passing urine and also obstruction to the passage of urine, mootrakricchra is also found in many diseases as one of the important symptom. Kricchra is the principle symptom in the mootrakricchra even though mutra vibandha i.e.obstuction to urinary flow may also occur, dysuria is the pain full passage of urine6.

Mainly there is 8 types of mootrakricchra1A,when different doshas vitiated or combined vitiated by their swaprakopaka karnas they goes to vasti and produces vikruti in mootramarga then person passes urine with very difficulty with pain i.e. called mootrakricchra1A.

In mootrakricchra the main symptom is painfull burning micturation. The line of treatment of mootrakricchra is basti because of vata pradhan vyadhi1Aand margasamipatvam, hence the study is intended. Here we are selected drakshdi niruha basti1A and rasnadi asthapana basti2A for management of mootrakricchra in our clinical trial.

6.2NEED FOR THE STUDY:-

1.  Every human being may suffer with dysuria in their life span, upto 50%of woman have a dysuria at some time. The prevalence of urinary diseases in women is about 3% at the age of 20years ,increasing by about 1% in each subsequent decade.7

2.  Surveys in recent year have shown that antibiotic resistance in this bacterium is increasing, making recurrent infection increasingly difficult to treat the diseases pertaining to urinary system8.

3.  A number of drugs are described in ayurveda for the treatment of urinary disease in classics. On the basis of some empirical experience, so it is need to conduct planned clinical trials on basti chikitsa which is one of the line of treatment of mutrakricchra. The comparison of these two basti are considered in the form of yoga basti in cases of mootrakricchra.

6.3 REVIEW OF LITERATURE:-

Ø  Mootrkricchra is a disease described in almost all ayurvedic texts.

Ø  All the acharyas described the etiopathogenesis, symptomatology and treatment.

Ø  Charaka samhita.1

Ø  Susurut samhita.2

Ø  Ashtang hridaya.4

Ø  Madhava naidana.5

Ø  Nidana factors of mootrakricchra excessive exercise, strong medicine, ruksha madya sevana, excessive sexual indulgence, riding over fast vehicle, meat of anup animals etc leads to moootrakricchra1A.

Ø  When different doshas are vitiated or combined vitiated by swaprakopaka karana they goes to vasti pradesh and produce vikruti in mootra marga then person passes urine with very difficulty with pain i.e. called mootrakricchra1A.

Ø  Drakshadi niruha basti is mentioned in charaka samhita siddhi sthana bastisutriya siddhi adhyaya1B.

Ø  Rasnadi asthapana vasti is mentioned in sushruta samhita chikitsa sthana niruhakrama chikitsa adhyaya2A.

6.4 Objectives of the STUDY:-

1.  To study the disease mootrakricchra in detail.

2.  To evaluate the effect of drakshadi niruha basti on mootrakricchra1B.

3.  To evaluate the effect of rasnadi asthapana basti on mootrakricchra2A.

4.  To compare the efficacy of drakshadi niruha basti and rasnadi asthapana basti on mootrakricchra

MATERIALS AND METHODS :

7.1 SOURCE OF DATA:-

1.  Patients will be selected from Sri Siddharoodha Charitable Hospital, attached to N.K.J.Ayurvedic Medical College and P.G. research Centre Bidar, Karnataka.

2.  Ayurvedic Classics, Journals, magazines, seminars, conferences, digital library and web sites.

3.  Materials – raw drugs will be collected from the Ayurvedic pharmacy under the supervision of Dravyaguna specialist of N.K.J.A.M.C and P.G.Center.

4.  Rasashala attached to college for preparation of medicine under the supervision of Rasashastra expert. .

7.2 METHODS OF COLLECTION OF DATA:- The study is a clinical study, in which 30 patients (including both groups) will be selected on the basis of simple randomized sampling procedure according to inclusive criteria.

RESEARCH DESIGN

All 30 patients will be distributed in 2 equal groups such as -

·  Group- A: - 15 Patients will be administered yoga basti chikitsa by drakshadi niruha basti.

·  Group-B: - 15 Patient will be administered yoga basti chikitsa by rasnadi astahpana basti.

Before starting treatment thorough counseling of patient and brief explanation regarding the procedure of Basti karma will be done, Patients of both the groups will be advised to stay in specific diet chart during the treatment.

Drakshadi niruhadi basti and Rasnadi niruhabasti are to be administered in the scheme of Yogabasti.

In both the groups patients will be advised to follow Parihar Vishayas (Dietic rules & regulations)

Follow up:

There will be two follow ups conducted i.e. 1st day soon after the treatment and 2nd will be 16th day after the treatment.

METHODOLOGY:-

The progress will be noted on the basis of assessment parameters (both subjective and objective) before treatment & after treatment in a specially prepared case sheet.

STATISTICAL ANALYSIS:

The observation will be analyzed critically & scientifically by employing some statistical tests between the groups and also within the group with suitable statistical analysis.

SELECTION CRITERIA:-

INCLUSION CRITERIA:-

1)  Patients within the age group of 20-60 years will be taken irrespective of sex socioeconomic status etc.

2)  Patient suffering from mootrakricchra on the subjective and objective signs and symotoms explained as per classics.

3)  Patients eligible for yoga basti.

EXCLUSION CRITERIA:-

1)  Patient suffering from diabetes mellitus/hypertension/acute and chronic renal failure/aids and other chronic disorders.

2)  Patient with traumatic injury to urethra.

3)  Patients suffering from obstructive pathology.

4)  All the condition in which Basti karma is contraindicated.

ASSESSMENT CRITERIA:

Assessment will be done based upon the improvement in the signs and symptoms of patient mentioned in subjective and objective parameters.

PARAMETERS FOR ASSESSMENT:-

SUBJECTIVE PARAMETERS:-

1)  Painful micturation.

2)  Burning micturation.

3)  Scanty micturation.

4)  Turbid micturation.

5)  Frequency of micturation.

6)  Urgency of micturation.

OBJECTIVE PARAMETERS:

1)  Urine routine examination.(Presence of RBC and Pus cells and Acidic, PH values and other microscopic abnormalities.)

7.3 INVESTIGATIONS REQUIRED:(if necessary)

1) Urine culture before and after treatment.

2) Biochemical analysis; Serum creatinine and blood urea, USG, IVP if necessary.
