6.BRIEF RESUME OF THE INTENDED WORK

6.1 NEED FOR STUDY

Chronic suppurative otitis media is one of the most common health problem in India.Complications in CSOM is the account of poverty, ignorance and non-availability of facilities on time. Overall complication rate in otitis media has fallen from 3% in pre-antibiotic era to less than 1% in the antibiotic era. So this study attempts to throw a light on the different extracranial complications of CSOM and the importance of early diagnosis and treatment so as to decrease the morbidity and give safe, dry and functioning ear.

6.2REVIEW OF LITERATURE

1.  Gabriel Fallopious (1523-62)discovered the facial nerve canal and first observed aural polyp and treated it.

2.  Giovauni Morgagni (1682-1771) was the first to describe that brain abscess is a complication of ear infection.

3.  William Wilde 1853 described the sub-periosteal abscess following ear infection and drained it with a post-aural incision.

4.  Herman and Schwartz 1870 were the first to report the opening of mastoid antrum for middle ear infection management and was later called as SIMPLE OR SCHWARTZ MASTOIDECTOMY.

5.  Gluseppe Gradenigo in 1967 described the sixth nerve palsy and pain in trigeminal distribution with suppurative diseases of petrous apex later it was known as Gradenigo's syndrome.

6.  Bezold F.S. 1908 described the pus from mastoid to track deep to sternomastoid muscle after eroding tip of mastoid later known as Bezold's abscess.

7.  In 1930 - 1937 Kopetsky, Almor and Eagleton, Frenckner, Ramdier and Lempert, Farrior described routes of drainage of abscess of petrous apex.

8.  In 1958, Heerman described temporalis fascia can be used as a graft in reconstruction of tympanic membrane.

6.3OBJECTIVES OF STUDY

1.  To study the risk of extracranial complications arising in cases of CSOM.

2.  To study theextracranial complications of CSOM with respect to age,sex and socio-economic status.

7.MATERIALS AND METHODS

7.1SOURCE OFDATA

Patients attendingthe ENT department presenting with purulent discharge from ear at Sri.Venkateshwara ENT Institute and Bowring and Lady Curzon Hospital, attached to B.M.C. during the period of study from Sept. 2008 - June 2010.

7.2 METHOD OF COLLECTION OF DATA

1.  Proper history taking with the help of a proforma.

2.  Clinical examination.

3.  Relevant investigation (CT Scan of temporal bone, X-Ray of the mastoid).

4.  Appropriate medical or surgical interventions.

Sample Size - A minimum 50 cases will be taken for study.

7.3 INCLUSION CRITERIA

1.  Patients of all age groups and both sexes.

2.  Cases of CSOM.

7.4 EXCLUSION CRITERIA

1.  Cases with intracranial complications of CSOM.

7.5 DOES THE STUDY REQUIRE ANY INVESTIGATIONS OR INTERVENTIONS TO BE CONDUCTED ON PATIENTS OR OTHER HUMANS OR ANIMALS ?

Yes, the patients will be subjected to the following investigations :-

1.  CT Scan of the temporal bone.

2.  X-RAY mastoid lateral and oblique views.

3.  X-RAY neck and chest.

4.  Audiometry

This study is conducted only after informing the patients and after obtaining their written consent for any surgical intervention.
