6. BRIEF RESUME OF INTENDED WORK:
NEED FOR THE STUDY :
Madhumeha is a disease of antiquity. The reference pertaining to Madhumeha are available in some of the most ancient texts including Charaka samhita. Hippocrates has also made specific reference to Diabetes.
The incidence of Diabetes has shown an alarming increase around the world. The developing countries following in thefoot steps of developed countries in life style have also started recording an increased incidence of Madhumeha.
Diabetes mellitus which was once known as Richman’s disease has now become Common man’s disease. As per WHO, 150 million peopleare reported to be suffering from Diabetes. The incidence of Diabetes mellitus in India is rising very fast almost three times to that of what is recorded in world1.
A series of epidemiological studies carried out by Diabetes mellitus Research Centre in Chennai has stated that, the prevalence of Diabetes mellitus had steadily increased among urban Indian adults from 5.2% in 1984 to 13.9% in 2000. It is estimated that there are about 33 million adults suffering with Diabetes mellitus in India. This number is expected to increase to 57.2 million by the year 20251.
The management of DM still remains unsatisfactory in spite of many advanced medicines and pathological investigations. Almost all treatment aspects focus towards increased blood sugar and use of oral or parental hypoglycemic drugs. Drug intolerance, hypersensitivity, resistance to insulin, the danger of acute and chronic complications, the fear of hypoglycemic series make it all to search out safe, effective, long lasting treatment. Such treatments which breaks the pathological process of DM & prevent its complications are available in Ayurveda.
An effort is made here in to understand the Samprapti of Madhumeha in a better manner and also to understand the process of Samprapti vighatana by Nisha Triphaladi Yoga.
6.2 REVIEW OF LITERATURE:
The knowledge of nidana is essential to know the nature of disease, its sadhyasadhyata & chikitsa.
- In charaka samhitha disease Madhumeha described in nidana sthana 4th chapter& in chikitsashana 6th chapter. 2
- In Sushruta samhitha disease has been described in nidana sthana 6th chapter in Chikitsa sthana 11th chapter and 13th chapter.3
- In Astanga hridya disease has been described in nidana sthana 10th chapter & in chikitsa sthana 12th chapter.4
- In madhava nidana the description of disease in 33rd chapter.5
- In Yoga ratnakara Madhumeha mentioned in 34th chapter.6

6.3 OBJECTIVE OF THE STUDY :
1)The present study is intended to understand the aetiopathogenesis of Madhumeha w.s.r. to Non Insulin Dependent Diabetes Mellitus Type 2.
2)The study is aimed to understand the process of samprapti vighatana of Madhumeha through the use of Nisha triphaladi yoga.
7.METHODOLOGY:
7.1 SOURCE OF DATA:
For the present study patients will be randomly selected from the OPD and IPD of Shri Jayachamarajendra institute of Indian Medicine Hospital(Teaching Hospital of G.A.M.C) Bangalore.
7.2 COMPOSTION OF TRIAL DRUG:
NISHA TRIPHALADI YOGA:6
INGREDIENTS:

Preparation of Medicine:
All the ingredients are collected after proper identification. The useful parts and the ratio of the individual ingredients are as per classical reference. Preparation will be undertaken with good manufacturing practice.
7.3METHODS OF COLLECTION OF DATA :
Both male and female patients for the age groupof 40 to 60 years attending the OPD and IPD of Shri Jayachamarajendra Institute of Indian Medicine Hospital ( Teaching hospital of G.A.M.C,) Bangalore.
7.3a STUDY DESIGN
- Patients fulfilling the inclusion criteria selected for the study will be divided into 2 groups. Group A and Group B with 20 patients in each group
- Patients in group A will be administered Nisha triphaladi yoga Hima 1 pala in a day and also be kept on appropriate Pathya Ahara and Vihara.
- Patients in Group B kept as Control Group only on Pathya Ahara and Vihara.
- The Findings are statistically analyzed by using Chisquared Test.

7.3b SAMPLE SIZE
A Minimum of 40 patients. 20 patients in each group selected for the present study.
7.3c Inclusion Criteria
Patients satisfying following criteria are selected for the study.
1)Patients having classical features of madhumeha like
Prabhuta mutrata
Avila mutrata
Pipasa adhikya
Karapada daha and suptata
2)Patients are between age group of 40-60yrs irrespective of sex ,religion & occupation etc selected for study.
3)FBS ranging- 126-200 mg/dl
PPBS ranging–180-250mg/dl
7.3dExclusion Criteria
- Patients with other systemic disorders like Cardiac illness, Bronchial Asthma, HTN, Endocrine disorders and Pregnancy.
- Age group less than 40yrs & more than 60 years.
- Patients havingSahaja prameha.
- Patients with complications of NIDDM Type 2
7.3e Diagnostic criteria
Patients are diagnosed clinically on the basis of signssymptoms and also through laboratory investigations as mentioned under inclusion criteria.
7.3 f Posology
NISHATRIPHALADI YOGA HIMA – 1 pala
7.3gDuration of the Study - 48 Days
7.3hFollow up – 15 Days
7.3iAssessment Criteria
The Assessment will be made on the basis of the following subjective and objective parameters.Conclusion will be drawn on the basis Chi-squared Test &Students t test.
