Infrastructure Status:

1.  Total land in the possession of the institution (supply copy of Farad)

2.  Whether Change of Land (CLU) of the land obtained from the competent authority (i.e. Chief Town Planner Punjab)? if yes, supply copy of CLU.

3.  Whether the land of the institution falls within the Municipal Corporation (MC) or outside MC area?

4.  Total covered/constructed area for all purposes.

5.  Whether building plan was approved from competent authority? if yes, supply year wise breakup of the construction of the building.

6.  Whether there are boundary wall with gated security?

Facility Status:

1.  Whether there are barrier free environment?

2.  What is the source of drinking water? Whether it is potable?

3.  What are the safety provisions in case of contingency and whether the NOC has been obtained from District Fire Brigade Officer regarding safety from the fire?

4.  Is there proper sewerage disposal system and the institution has obtained NOC from Punjab Pollution Control Board?

5.  Whether the Institute has obtained building completion certificate from the competent authority (i.e. from the department of Town & Country Planning) before the use of the building?

6.  Whether there is proper parking palace inside the boundary of the college/institution?

7.  Whether there is separate hostel facility for boys and girls? Give detail.

Pass out and Placement Status

1.  Stream wise list of students passed out during 2010-11 to 2014-15and how many students has been placed out from the passed out students.

Anti Ragging Status:

1.  Whether Anti Ragging Committee constituted, if yes provide list of members?

2.  Whether Anti Ragging Squad constituted? if yes, provide list of members of squad alongwith reports of the squad?

5 / Are the non teaching staff being paid as per Punjab Govt. scale, if yes, supply the bank salary slips for the month of March 2015?
6 / What is the fee structure being charged? Whether the rates of tuition fee being charged from the students are as per schedule approved by the university?
7 / Component wise detail of fee charges (other than tuition fee) being charged from students on annual basis. (attach the separate sheet if required)
8 / What amount of security is being charged from the students and how many students are there who had left out the college but did not claim the refund of the security?
9 / No. of drop out of students and the detail of charges received from them but not reimbursed to them at the time of drop out.
10 / Whether Reservation are being provided as per Reservation policy?
11 / Actual strength of students against approved intake.
12 / Give procedure of admission in brief and admission made after counselling. Is there any type of Management/NRI Quota in Admission, if yes, what percent? and whether the college/institution is collecting any capitation fee donation from the students.
13 / Whether the accounts of the Institution are being audited by Charted Accountant (CA), if yes, supply copy of the final accounts for the last five year (2010-11 to 2014-15)?
