Quarterly Achievement Incentive
-Quarterly Pay-outs vs yearly evaluations solely based on YOUR performance.
-Each employee can earn up to 12 points per year to obtain a maximum of 6% of your projected yearly salary. Quarterly incentive bonuses will be based on the employees performance every 3 months as assessed by the Office Manager and Physicians.
-Every employee will be eligible after a 90-day probationary period.
Performance is broken down into 3 categories:
-Individual Performance
-Departmental Performance
-Company Wide Performance
Each quarter, for every employee, the 3 categories above will be evaluated. An employee can earn 1 point for each category each quarter. Quarterly achievement bonuses will be determined by the number of points earned each period.
In order to earn 1 point per category, ALL criteria MUST be met!!
INDIVIDUAL PERFORMANCE
_____Attendance. > 98% of time in office and if not, provides > 48 hours in advance notice.
______Behavior demonstrated/CONSISTENT enthusiasm, professionalism, dedication to your job. Helpful, friendly behavior demonstrated > 98% of time towards patients, co-workers and visitors.
______Appearance. Adheres to the dress codes to include lab coats and name
tags worn > 98% of time.

DEPARTMENTAL PERFORMANCE
FRONT DESK/CHECK-IN AND CHECK-OUT
_____MD schedules on a quarterly basis are filled > 80% of time.
_____No Show rate per physician per Quarter is < 7%.
_____Loose material to include progress notes, H&P’s, Rx refill requests are to be filed under the correct tabs and within 48 hours of receipt.
_____Accuracy of patient demographics and insurance information entered the FIRST time > 98% of time.
_____All phone calls are being answered within 3 rings.
_____Dictation is uploaded/downloaded within 24 hours from DOS > 98% of
time.
_____Charts for upcoming appointments are being pulled 5 days or more in advance > 98% of time.
_____For NP appointments, pertinent records including records from previous rheumatologists, relevant lab reports and X-rays, are obtained at least 1 day PRIOR to the scheduled appointment.
_____For follow-up visits, results of lab tests, X-rays, consultations, etc., are in the chart PRIOR to the scheduled appointment.

MOA’s/LPN/RN
_____Nurse note taken to include 3 vital signs on all patients to include review of meds and write needed Rxs for visit with proper quantity (30 or 90 day supply), reason for visit documented to include specific concerns for that day > 90% of time.
_____Patient prepped as necessary for exam or procedure. (ie. WI for foot pain or bilateral knee pain)
_____Keep biohazard containers compliant > 98% of time.
_____Keep clinical supplies stocked to includeLab supplies current > 98% of time.
_____Speak with drug reps to obtain any needed samples and patient assistance material when applicable > 98% of time with a helpful, friendly behavior.
_____Return NURSE SPECIFIC patient messages received by 5 pm that day > 98% of time on calls received by 4 pm.
_____Ensure patient rooms are properly stocked and cleaned between patients and at end of each day > 98% of time.
_____Ensure specimens have proper labels on tubes > 98% of time.
_____Clearly give and/or reinforce new instructions regarding treatment > 98% of time.
_____For any new Biologic patient, should have PPD placed and arrangements made for reading, @ time of biologic order > 98% of time.
_____Discard opened injectables that are over 30 days.

MANAGER/BILLING SUPERVISOR
_____Ensure individual quarterly reviewsare held with all staff members prior to due date >98% of time.
_____Ensure month end close is done by month end to include sending out statements > 98% of time.
_____Ensure staff has all necessary tools, supplies and equipment to do their job by inquiring with the staff weekly.
_____Review infusion schedules to ensure all necessary authorizations have been obtained for upcoming appointments > 98% of time.
_____Attend quarterly Managers Meetings in to network and improve practice profile.
_____Effectively manage personnel to include creating work schedules, holding monthly staff meetings and arrange vacation coverage when necessary.
_____Generate monthly reports as required and/or requested by physicians, bookkeeper and accountant.
_____Effectively communicate relevant findings of personnel development to the physicians.
_____Promote an atmosphere of professionalism, as well as, compassionate and courteous patient care.
_____Assist with marketing and promotion of practice through personnel initiatives, as well as, carrying out physician requests.

_____Average quarterly score on Patient Satisfaction Surveys, 4 or >.
_____Profit – Average total receipts > $775,000.00/Quarter.
_____Average monthly A/R per Quarter is < $250,000.00
_____Overhead expense is < 80 % per month.
