This application in conjunction with the Qualified Reporting Entity Service Agreement (QRESA) is for approval to allow the California Independent System Operator (ISO)to perform the services required of the Qualified Reporting Entity (QRE)reporting meter data associated with renewable energy on behalf of the Generator Owner on a monthly basis. The ISO will report such data using the Western Renewable Energy Generation Information System (WREGIS) administered by WECC per their program requirements.

II. Conditions Prior To Submission of Data to WREGIS

By completing this application, the Generator Owner or Generator Agent Applicant acknowledges that each of the following conditions must be met before the ISO will submit data to WREGIS on behalf of the Generator Owner or Generator Agent Applicant pursuant to the QRESA:

A.The resource must have a certificate of compliance issued by the ISO.

B.The effective date,associated with the beginning of data submission to WREGIS, will be the later of the following dates:

1. The date of execution of this QRE Service Request Application.

2. The resource effective date as established by ISO Meter Engineering and Analysis.

Note: The ISO will not submit meter data prior to the effective date determined by Condition B above.

III.Additional Information and Obligations

By completing this form, the Generator Owner or Generator Agent Applicantrepresents, warrants and certifies under penalty of perjury and subject to any applicable penalties under the ISO Tariff that:

a.The information submitted within this application is correct and accurate.

b.The Generator Owner or Generator Agent Applicant is eligible to participate in WREGIS as a qualified renewable resource.

c.The Generator Owner or Generator Agent Applicant has an approved and filed Terms of Use Agreement with WREGIS

d.The Generator Owner or Generator Agent Applicant has an MSA/ISOME with the CAISO and is an actively ISO polled Metered Service Entity.

e.The Generator Owner or Generator Agent Applicant has read and agrees to be bound by the QRESA and the QRE Service Request Process document, as each may be amended from time to time, the terms and conditions of which are incorporated by reference herein. f.Upon receipt of WREGIS notification of QRE meter data submittal (WREGIS Accepted), meter data should be reviewed and approved within ten (10) days. The Generator Owner of Generator Agent Applicant agrees to notify the ISO QRE Coordinator of any data discrepancies within ten (10) days of file acceptance within WREGIS.

Please Note:The resubmittal of meter data will not occur unless obligation “F” listed above has been met.

g.The Generator Owner or Generator Agent Applicant agrees to provide further information to the ISO that the ISO may deem necessary to approve the Request and maintain approval on a continuing basis.

h.The Generator Owner or Generator Agent Applicant will promptly notify the ISO of any material changes in any of the above sections as soon as it becomes aware of such changes.

i.The Generator Owner or Generator Agent Applicant consents to allowing ISO to independently verify any of the information provided herein and to cooperate with ISO in providing such information.

j.Although ISO has been granted the right to verify any information contained in this form, Generator Owner or Generator Agent Applicant acknowledges and agrees that ISO may rely solely on the information contained herein without verification.

k.The Generator Owner or Generator Agent Applicant has read and agrees to be bound by the ISO Tariff, as it may be amended from time to time.
