The district must submit a separate application for each candidate. If the same OEE will be working with more than one school in a district, the district should complete one application and indicate the assigned schools.

n. For candidates with New York State administrative experience: Experience as a leader of a school with an accountability status of “In Good Standing”. / 1234NA
I understand that the New York State Education Department (NYSED) requires OEEs to attend NYSED mandated training prior to participating on SED led IIT visits or on District led DTSDE visits. OEEs must participate in additional training as directed by NYSED. By signing this form, I agree to comply with NYSED training requirements.
