The Appointment Change Form is used for the following circumstances involving current employees: Promotion (involves $$), title change, reassignment, transfer. Changes involving Academic Professionals require a PAPE form.

Please note that mid-year promotions with salary increases must be justified under extraordinary circumstances involving a clear change in duties, a salary inequity, or a counter offer.

Attach the proposed appointee's current resume or c.v. and an organization chart if the unit is undergoing a reorganization (current and proposed charts). If this is a request for an Academic Professional, please fill out and attach the PAPE form- Appendix C on the LAS Hiring website for the proposed title.

LAS will review the request. If approved, LAS will forward to ODEA for review. ODEA will contact the College with questions. The College will be in contact with the Department for any clarifications needed.
