This Addendum forms part of and modifies Proposal Documents dated, February 18, 2015, with amendments and additions noted below.

The following questions were received prior to the deadline for questions:

  1. Please confirm that no bid bond is required with the submission of the JOC proposal.

Correct – a bid bond is not required with this proposal.

  1. Please confirm that, if offerors submit a Proprietary Information Statement to identify and protect select information that is considered confidential in accordance with the Texas Public Information Act (Texas Government Code, Chapter 552.001, et seq.), the Proprietary Information Statement will be excluded from the page limitations.

  1. Paragraph 1.5.1.1 on RFP p. 4 notes that eight identical copies of the proposal are required (along with the required electronic copy on CD). Please confirm that offerors may submit one original hard copy (with original signatures) plus seven additional hard copies to meet the requirements for a total of eight copies.
    That is correct.
  1. Paragraphs 4.2.1–4.2.2 (RFP p. 12) note that proposals should be spiral bound together, separating each Criteria Response in Section 3 by tab. However, Paragraph 4.1.2.1 (RFP p. 12) notes that proposals should be submitted as two separate documents. Please address the following:
  1. Is it acceptable to submit the Criterion One documents (Section 6 – Pricing Proposal and Section 7 – Execution of Offer) in a sealed enveloped separate from the Criterion Two documents and without spiral binding (while the Criterion Two response is assembled with spiral-type binding)? Or does UTHealth prefer that Criterion One and Criterion Two be assembled together as one unit with spiral-type binding and separated by tabs?
    Proposers can submit their Pricing Proposal in a sealed envelope separate from the Criterion Two documents and without spiral binding.
  2. Please confirm that offerors should submit eight copies each of Criterion One and Criterion Two. (We ask because sometimes owners prefer to receive fewer copies of the price proposals.)
    We don’t need eight copies of the pricing portion. You can submit just one copy of the Pricing Proposal.
  1. Please confirm that offerors may submit the files for both Criterion One and Criterion Two on the same CD.
    Yes, that is correct.
  1. Please clarify the annual maximum contract value this new contract. Is it limited to $1 million per year, as indicated in Paragraph 2.01 of the sample JOC Agreement (Exhibit C)? Or is there an annual maximum amount for the JOC program as a whole, to be shared among the three contractors?
    It is $1 million per year independent of the other contracts.
  1. Please indicate the total value of work awarded to date to each of the two contractors that currently hold job order contracts with UTHealth.
    During a competitive solicitation process, we do not provide statistical data on current contracts.
  1. Please indicate the following for work accomplished through the UTHealth’s two ongoing job order contracts to date:
  • The approximate percentage of projects awarded with values less than $50,000
  • The approximate percentage of projects awarded with values between $50,000 and $100,000
  • The approximate percentage of projects awarded with values between $100,000 and $250,000
    During a competitive solicitation process, we do not provide statistical data on current contracts.
  1. Approximately what percentage of work under this contract does UTHealth anticipate will occur in wet lab facilities?
    20%
  1. Approximately what percentage of work under this contract does UTHealth anticipate will occur in patient care facilities on the UTHealth campus (e.g., facilities for patient services offered at the School of Dentistry)?

  1. Will work under this contract involve performance in affiliated patient care facilities – such as the Memorial Hermann Hospital System, clinics, and other affiliated institutions – in addition to the UTHealth’s primary academic and research facilities?
    UTHealth only has outpatient clinics and one psychiatric hospital.
  1. If the answer to Question #11 above is yes, approximately what percentage of work is expected to occur in these affiliated patient care facilities?

  1. How does UTHealth intend to award projects among the three contractors holding job order contracts following this procurement? For example, does the type of work (e.g., work in wet labs) or project size (e.g., projects over $100,000) affect how projects are assigned among the contractors?
    Based on qualifications for the type of project and past performance on other UTHealth projects.
  1. Please confirm that, in addition to the elements excluded per Paragraph 4.1.2 on RFP p. 12, the following elements also do not count against the 50-page limitation:
  • Cover letter
  • Resumes for key personnel
    We will not count these items against the 50-page limit.
  1. Paragraph 4.4.1 on RFP p. 13 references the HUB Subcontracting Plan with regard to pagination requirements. However, it is our understanding (from Paragraph 1.12.1 on RFP p. 6) that no HUB Subcontracting Plan is required with submission of the JOC proposals. Please confirm that this is correct.
    Correct – a HSP is not required with this proposal.
  1. There is a note preceded by double asterisks (**) on RFP p. 27 of 27, following the “Miscellaneous” section of the Respondent Questionnaire (Section 8). Please clarify if this note is related to a specific question/requirement on the Respondent Questionnaire or if offerors are simply to be aware of the information provided in the note, in general.

  1. It is our understanding (from Paragraph 2.2 on RFP p. 8) that UTHealth intends to make a single award to add a third vendor to the JOC program through this procurement, and therefore contractors holding the current JOCs are not eligible for award of this new JOC. Please confirm that this is correct.
    That is correct.
  1. Please clarify which page(s) of Exhibit C, sample JOC Agreement, constitutes the Agreement’s Exhibit A, Request for Proposal form.
    There is a mistake on this table of contents. Exhibit A, Request for Proposal form should be omitted.
  1. Please clarify which page(s) of Exhibit C, sample JOC Agreement, constitutes the Agreement’s Exhibit B, Project RFP Response form.
    Pages 15 and 16 of the sample agreement.
  1. Item 14 in Section 6.2 Coefficient Development states that we are to include permits, licenses and fees in our Coefficient. Will work under the JOC contract be permitted by the City of Houston?
    COH permits are not required for renovations with-in state/UTHealth owned buildings.
  1. Can you provide us with a range of approximate volume of work that has been historically produced for contract holders of the JOC at the UT Health Houston campus? We understand that the volume of work given is based on contractor performance, but a range would help in developing our modifier rate.
    It varies quite a bit from year to year and across companies. Unfortunately during a competitive solicitation process, we do not provide dollar spend on current contracts.
