Golf Tournament Entry Form
Date: September 6, 2014 – 10:00 am shotgun
Course: Riverside Golf Club, 1451 NW Airport Rd, Chehalis, WA 98532.
Entry Fee: $75.00 per player or $300.00 per foursome includes riding cart, range balls, entry into the
hole-in-one contest, lunch and prize drawing after the tournament. No Refunds (it’s a fundraiser!)
Registration begins: 8:30 am
Format: 4 person scramble. No golf handicap needed.

Hole In One Hole Contest on #13 to win a 2015 Chrysler 200 Limited – Valued at $25,000. PLUS the other three Par 3’s will receive lesser prizes for a hole-in-one. IF NO ONE GETS A HOLE IN ONE ON #13: A drawing will be held from all of the entries and the winner will receive a $500 gift card to be used at Riverside Golf Club.
Golf Prizes:
1. Closest to the Pin on each Par 3. Men & Women’s divisions for a dozen golf balls.
2. Low Team Score: each team member receives a $100 gift certificate to the Riverside Pro Shop.
3. 2nd Low Team Score: each team member receives a $50 gift certificate to the Riverside Pro Shop.
4. Highest Team Score: A one hour group lesson from Riverside’s Pro – Austin Abbott. Value –$100
The following will be available on the day of the tournament for an additional fee:
1.Beat the Pro Straight Drive: Prize drawing for a new Calloway X-Hot Driver & Fairway Wood.
2.Mulligan: One per player to be used anywhere on the course.
3.Paint Stick. Use a paint stick to extend and make your putt.
4.Toss Your Ball – Extend your golf shot. Example – hit an approach shot short of the green then toss your ball close to or in the hole to extend your shot.
5.Raffle drawing - variety of prizes donated by local businesses.
6.50/50 Split The Pot Putting Contest. $5.00 for two putts on the putting green when you make the turn. Closest to the pin wins half of the pot. Ties will be putted off on the 18th green in an elimination format..
