Introduction: The purpose of this report is to provide information about the assessment actions, data, and events that occurred each academic semester. These reports allow departments and programs to provide updates on how their assessment plans are being implemented, improvements to student learning, and plans for on-going assessment strategies and processes.

The report will be submitted to the Office of Academic Affairs and will be available to appropriate accrediting agencies. It is recommended that each program use their assessment report and results to acknowledge achievements of student learning as well as to identify potential areas for future curriculum improvement. Please email this completed form as an attachment to by December 16, 2011.
