Define Outcomes in Measurable Terms:
The Management Team will develop and define outcomes in measurable terms. All outcomes will be written in the same format and will include outcomes that measure efficiency, effectiveness, access and satisfaction. If appropriate, a progress measure must also be in place.
Write Procedures For Measuring Data
Along with developing and defining the outcome measures, the Management Team or their designee will prepare a written procedure on how data will be collected, measured, and who is responsible for collecting the data for each outcome measure.
Computerize QA Information
All strategic plan measures will be entered and tracked in the computer.
Train Appropriate Personnel
Managers/Supervisors will inform all team members or appropriate staff about the outcome measures they are responsible for in their area. In addition, the manager/supervisor will make sure each staff person receives training on how to accurately collect and record data.
Collect Data Consistently
It will be the ultimate responsibility of the Management Team to insure that outcome measurement data is collected and reviewed on a regular and consistent basis. Data should be reviewed by team members throughout the month to determine if they are on target to meet their goals.
Report Outcomes Monthly
All outcome measurement and satisfaction data will be tabulated at least quarterly by the person
designated for each area. The results will be reported back to the Management Team. Quarterly
reports will be written in the same format and done in a timely manner.
Management Review or QA Outcomes and Action
All outcomes and data will be reviewed by the Management Team on a regular basis. Information will be used to compare current program results with identified measures and goals and make necessary changes in service methods and delivery to better accomplish identified goals of the agency.
Communicate Results
Once all of the data has been received from each of the service areas, the results may be given to the PR department for communication via the Progress Weekly, Progress News newsletter, and other appropriate means. (ie: quarterly/annual report, Business & Progress flyer, correspondence, etc.) On a quarterly basis, this information will be reviewed by the Management Team.
Progress Industries Program Evaluation System Description
The PI program outcome system continually gathers information throughout the year. Data comes from financial reports, satisfaction surveys, job placement statistics, intake data, persons served attendance, and staff turnover data. This information is reported monthly by each source and recorded in the current program evaluation spreadsheet. On a quarterly basis the information is summarized and reported and reviewed by the management team and the board. This information is also shared throughout the year with employees and other stakeholders via electronic and paper newsletters, our website and placement of news articles.
The system was developed from and around our mission, vision and values.

The evaluation system is also focused on the programs and services that are accredited by CARF: Organizational Employment, Community Employment, Supported Living, Community Housing and Community Integration, Aspire to Excellence (overall agency and governance.) Each of the program/service areas measures are the objectives for each program/service. Each area has an effectiveness, efficiency, access, and satisfaction measure/objective. Each measure identifies the person responsible for collecting and entering the data into the spreadsheet. Currently the Communications Director is charged with summarizing and presenting the information to the management team and the board. The public relations staff uses the summarized information to share with other stakeholders, staff and the public.
