JOB PURPOSE
To provide support strategies targeting community members aged 65 years and older to maintain good quality health, prevent avoidable hospital admissions and ensure equitable access to health services. The role will ensure the coordination and integration of primary health care services across the region based on activities on priorities highlighted in annual needs assessments and outlined in the ESML annual plan.
DUTIES:
Under the guidance of the Manager, Population Health the incumbent will be responsible for;
- Develop, implement and evaluate region-wide primary health care strategy to address the following priority areas;
-Needs of residents of aged care facilitieswithin the region
-Strategies to address primary and secondary falls injuries in the ESML region
- Source relevant data to inform planning, strategy design and reporting
- Explore and support opportunities for research and innovation relevant to aged care and residential aged care facilities
- Prepare relevant plans for priority areas including annual and project plans and ensure inclusion of best practice and population wide approaches are integrated
- Work collaboratively with external stakeholders to support the delivery of strategies for priority areasincluding; local health district, Non-government organisations, public and private organisations, consumers
- Work collaboratively with internal stakeholders to ensure integration and coordinated strategies for example;eHealth strategies, after hours, communications, practice and network support functions
- Coordinate a reference group/steering committee with representatives from relevant stakeholders to provide governance and direction regarding the relevant program areas
- Contribute to the development of resources to support the implementation and evaluation of the each priority including website, print and media resources
- Prepare reports and feedback in line with contractual arrangements and ESML internalprocedures including;12 month reports, Board reports, Team report and use of ESML knowledge management (CRM) system

- Appointee authorised to determine the need to make business related STD, mobile and tele/video conference calls.

Selection Criteria
Essential
- Tertiary qualifications in health, social sciences or relevant experience
- Understanding of the concepts relevant toaged care, residential aged care facilities and falls prevention and how they apply to primary health care
- Experience implementing and evaluating strategies to supportaged care and/or falls prevention and management
- Experience in project management including planning, implementation and evaluation
- Knowledge of and commitment to principles of best practice and how they apply to aged care and falls prevention
- Demonstrated organisation, interpersonal skills and effective written and verbal communication skills
- Ability to work independently and as part of a team
- Demonstrated relevant computer skills including using Microsoft Office programs and the Internet.
- Current NSW Drivers Licence
- Experience implementing health promotion strategies to support aged care and/or falls prevention
- Experience working with a range of stakeholders to deliver aged care and/or falls prevention strategies
