Procedures for Mental Health Services

Teachers discuss child/family social and emotional and mental health referral needs with their ED Coach; EHS & Home Visiting staff discusses with their Supervisors; Child Care Providers discuss with their Child Care Support Coordinator. Call and/or email Stacey Parent with questions or for problem solving situations. Stacey will determine which Consultant or Therapist will best meet child/family needs. Stacey may refer staff to speak with a MH Consultant for more specialized guidance or 1:1 reflective practice. Staff may request 1:1 reflective practice at any time. We are not a crisis program; however, families & staff should receive support within 1-3 weeks after (mh-6) is received. Staff follow-up on referrals and track in their Contacts, Inkind and Social Service Tracking.

Conscious Discipline: Observations home visits are requested through theMental Health Child/Family Request/Referral/Release – Sharing of Confidential Information (mh-6). Individual site/parent training is requested via the Request for Conscious Discipline Training form;please complete forms thoroughly. Both forms can be emailed or faxed directly to Lesa Rice. Classroom Mental Health Requests:Are requested through the Mental Health Child/FamilyRequest/Referral/Release – Sharing of Confidential Information (mh-6). Please complete (mh-6) thoroughly. Check the boxes for desired services; indicate the child/family insurance & if they have transportation. Teachers, Providers, ED Coach or Child Care Support Coordinator will email or fax the (mh-6) to Stacey Parent.Ask Stacey where to fax: either 231.922.9895 or 231.933.0013. Stacey will notify staff which Consultant or Therapist is designated and to expect a call from them for scheduling.

If the Mental Health Child/Family Request/Referral/Release – Sharing of Confidential Information (mh-6)involves a DECA and/or SPM-p:Discuss with parent/caregiver the purpose of the DECA and/or SPM-p, the child’s strengths & also concernsbefore giving them to complete. Obtain signatures on the(mh-6) for an Individual Child Observation. Keep the original DECA and/or SPM-pon site for the MH Consultant or Therapist. ADECA and/or SPM-p are completed after observing child for 4 wks. – The same for returning children.
Scoring DECA and/or SPM-p: Can be scoredby the ED Coach or the MH team.

An Individual Child Observation Request may involve a combination of support including home visits.

Teachers notify ED Coach & Providers notify Child Care Support Coordinator of referrals and child mental health planning meetings.

Mental Health referrals for Home-Based Support or Child/Family Counseling at an office: Are requested through the Mental Health Child/Family Request/Referral/Release – Sharing of Confidential Information (mh-6). Please complete (mh-6) thoroughly. Check the boxes for desired services; indicate the child/family insurance & if they have transportation. Teachers, Providers, EHS staff or Child Care Support Coordinatorswill emailor fax (mh-6) to Stacey Parent. Ask Stacey where to fax referral: either 231.922.9895 or 231.933.0013.Stacey will notify staff which Consultant or Therapist is designated andto either have the family call to schedule with a Therapist or to expect a call from a Consultant.

Referring families to Community Mental Health:Talk with Stacey about this process. The family still completes the (mh-6). Any family can call to see if they qualify, however, many of our families won’t qualify.
MH Referrals when a family self-refers with their own insurance paying: Complete (mh-6) for communication and feedback from Therapist on how we can support child/family.
Infant Mental Health: May be provided by MH Consultants, outside counselors or Community Mental Health/Pine Rest.
