Applicant/Student (hereinafter referred to as “Student”) has had the program fully and completely explained to him/her and agrees to abide by the terms listed in the Enrollment Agreement (hereinafter called “Agreement”) and by regulations and policies of Pro Barber College (hereinafter called “College”). This agreement and the catalog constitute the entire agreement between the College and the Student. The Student understands that if the Agreement is accepted by the College, it shall be deemed a binding contract.

This College reserves the right to withhold the Certificate or dismiss any student for maintaining poor academic standing, excessive absences, failure to comply with the regulations of the college as stated in current literature or for nonpayment of tuition and fees when due. Late charges of five (5%) percent will be added to any bill ten (10) days overdue. The Student will be liable for all late charges, reasonable attorney’s fees, and all other costs and charges necessary for the collection of all unpaid fees.

REFUND POLICY - Refer to the Tuition Refund Policy under Student Rights to Cancel Section of this agreement for more details.

ACKNOWLEDGEMENT – I have read and understand this enrollment agreement and I acknowledge receipt of an exact copy of same. This agreement contains all the terms. I acknowledge that no verbal statements have been made contrary to what is contained in this Enrollment Agreement. I also agree that the College may use my name and/or picture in any college literature or publications. I fully understand my rights and duties and agree to abide by them.

I UNDERSTAND THAT TUITION AND FEES ARE FOR THE RIGHT TO ATTEND THE COURSE IN WHICH I AM ENROLLED AND ARE IN NO WAY CONTIGENT UPON MY SATISFACTORY PROGRESS, PERSONAL SATISFACTION, OR PLACEMENT UPON GRADUATION. I FURTHER AGREE TO ABIDE BY ALL RULES OF PRO BARBER COLLEGE.

I understand and agree that this Agreement is executed for the entire period of the program in which I enroll.

TRANSFER OF CREDIT – The College does not guarantee any student that credits will transfer to another institution. The Student understands that the transferability of credits is dependent on the evaluating institution. Refer to State Mandated Disclosure.

EMPLOYMENT ASSISTANCE – The College does not guarantee any student employment upon graduation, however, Pro Barber College will assist the student in obtaining gainful employment. Refer to the School Performance Fact Sheet.

DISCLAIMER – In the event of a dispute arising out of this Agreement, the College shall be entitled to reimbursement of all reasonable attorney’s fees and costs expended by the College in the resolution of the dispute or collection of monies due; and in the event of litigation, the prevailing party shall be entitled to recover reasonable attorney’s fees and costs. Pro Barber College cannot be held liable if the Student is unable to obtain certification or licensing due to information not disclosed.

STUDENT RIGHTS TO CANCEL

ENROLLMENT WITHDRAWAL:

Students have the right to cancel the enrollment agreement. The College shall refund 100 percent of the amount paid for institutional charges, less Registration fee not to exceed two hundred fifty dollars ($250), if notice of cancellation is made through attendance at the first class session, or the seventh day after enrollment, whichever is later. STRF fees are refundable if a student withdraws during this period and given a full refund. STRF fees are not refundable after this cancellation period passes. Initial ______

WITHDRAWAL/TRANSFER:

Current students requesting to withdraw or transfer must inform the school in person or by letter. Any money owed to the school must be paid in full according to the original enrollment contract. A “Proof of Training” document will be provided to the student showing the documented hours accumulated at our school. You may retain this document for a later date if you cannot continue at this time. All student records that include hours accumulated in a California school must be kept for a minimum of 5 years. COURSE CANCELLATION POLICY:

If any courses offered are cancelled subsequent to a student’s enrollment, the school shall give the student a calculated refund depending on the hours already attended. Initial ______

RE-ENTRY POLICY:

A student may apply for readmission after a leave of absence and must pay a registration fee. If a student has taken an approved leave of absence and returned within the approved amount of time, he/she should not be subject to re-applying and paying a new registration fee. However, if he/she does not return from an approved leave of absence and is therefore withdrawn from the program, he/she would then be subject to this policy. Initial ______

TUITION REFUND POLICY:

Registration fee is non-refundable. At this time, the registration fee is $100.00, and the STRF is based on the course tuition. Please refer to the specific course for more details of tuition and fees.

Full refund is only applied if notice of cancellation is made through attendance at the first class session, or the seventh day after enrollment, whichever is later. STRF fees are refundable if a student withdraws during this period and given a full refund. STRF fees are non refundable after this cancellation period passes. After this 100 percent refund cancellation period has passed, the refund to be paid to a student shall be calculated by deducting the registration and STRF fees from the program’s total charges for a period of attendance and dividing this figure by the total number of program’s clock hours. The quotient is the hourly charge for the instruction. The amount of monies owed by the student for the purpose of calculating a refund is derived by multiplying the total hours the student had attended by the hourly charge for instruction. The amount of the refund to which a student would be entitled (after completing a period of days or weeks of instruction) will be calculated by deducting both the amount monies owed by the students and the non-refundable fees (registration and STRF) from the total charges of the program for the period of attendance (See Example 2 below). There will be no tuition refund for students who have completed 60 percent and more of the period of attendance of the program. The institution will pay or credit refunds within 45 days of a student’s cancellation or withdrawal.

If a student obtains a loan, the student will have to repay the full amount of the loan plus interest, less the amount of any refund, and that, if the student receives federal student financial aid funds, the student is entitled to a refund of the money not paid from federal student financial aid funds. Prior to signing this enrollment agreement, you must be given a catalog or brochure and a school performance fact sheet, which you are encouraged to review prior to signing this agreement. These documents contain important policies and performance data for this institution. The institution is required to have you sign and date the information included in the school performance fact sheet relating to completion rates, placement rates, license examination passage rates and salaries or wages, prior to signing this agreement. BARBER/HAIRSTYLING COURSE PAYMENT CONTRACT

Prerequisite for Barber/Hairstyling Course:

None

Length of Course: 75 weeks (Part time, 20 hours of attendance per week)

38 Weeks (Full Time, 40 hours of attendance per week)

ü  IF YOU OWE CHILD SUPPORT IT MAY INTERFER WITH YOUR BARBER LICENSE.

ü  YOU MUST BE 17 YEARS OLD AND HAVE A SOCIAL SECURITY CARD TO TAKE THE EXAM.

ü  THE SCHOOL WILL NOT PROVIDE YOUR HOURS UNTIL THE TUITION IS PAID IN FULL.

PAYMENT CONTRACT FOR BARBER/HAIRSTYLING 1500 HOUR COURSE

Please sign below that you agree to the minimum $300.00 a month tuition fee, not to exceed the schedule of total charges for a period of attendance which is $3,500.00. The tuition fee will be due and payable on or before the 5th of each month. There will be 5% late fee for payments received after the 10th of that month. Please understand your hours will not be provided to the State Board at graduation until balance is paid in full. If you request to do a Pre-application the balance must be paid by 1125 hours. These terms are subject to change.

CROSSOVER/REFRESHER COURSE PAYMENT CONTRACT

Completed 1600 hours of Cosmetology in an approved California cosmetology school and provide a “Proof of Training” Document s from the school or provide a California Cosmetology license whether expired or not.

Prerequisite for Barber Refresher Course:

A California Barber license whether it is valid or expired.
