Overview

General review of home modifications is to be provided annually (at the same time as review of any Domiciliary Equipment Services (DES) equipment items held by the client) to ensure that the modifications are being used appropriately and safely and do not present a safety risk to client or carer. The General Equipment, Home mod Reviewformwill be used by the reviewer to identify any issues with the home modification that may require a referral for a clinical review. The outcome of the review will be documented and referrals completed as indicated.
The review of home modifications is required to ensure ongoing safety of home modification use and to manage the risk of any injury to client or carer. The general review is completed annually to identify any problems with the modification or how it is being used by the client. If any problems are identified, this will result in a referral to a clinician to review the client’s use of the modification and to prescribe a replacement modification if DES is unable to repair modification. Ongoing repairs, maintenance and replacement of home modifications remains the responsibility of the home owner as agreed to at the initial prescription. Refer to Terms and Conditions of Installation of Home Modifications.
General reviews of home modifications are completed for current Disabilityand Domiciliary Care clients who have been provided with home modifications by the Department for Communities and Social Inclusion (DCSI).

Step / Description
1 / To complete the general review equipment screening tool
  • Complete the General Equipment, Home mod Reviewformeither as part of a home visit or over the phone with the client / carer when a scheduled review is due, or in response to contact from the client if there are concerns with the home modifications provided.
  • File completed screening tool in the client’s file and document any issues identified and actions taken.
Are there any issues identified with the safe use of the home modification?
If yes, go to step 2.
If no, go to step 3.
2 / To refer for clinical review of home modification
The service coordinator / key contact / accommodation manager completes a referral to the relevant discipline to complete a clinical review of the home modification. Referto procedure Completing clinical reviews of home modifications.
Go to step 3.
3 / To document that the general review of equipment items has been completed
  • Document the outcome of the general review of the home modification in the client’s file.
  • Save the completed review form in the client’s file (either paper or electronically).
