Directed Enhanced Services to be commissioned from GP contractors in 2013/14

From 1 April 2013 NHS England became responsible for commissioning primary care medical services. Practices will continue to be offered the opportunity to provide a variety of enhanced services nationally.

Four new enhanced services have been developed to promote quality and innovation in the delivery of general practice services. They are;

  • Risk profiling and care management scheme
  • Facilitating timely diagnosis and support for people with dementia
  • Remote care monitoring (preparation) scheme
  • Improving patients online access scheme.

Although all these services are commissioned by NHS England the risk profiling and care management scheme will be managed by Clinical Commissioning Groups (CCGs)as this DESis best driven locally and clinically. CCGs will be required to agree and manage monitoring arrangements with practices for this DES. We are currently working with CCGs to confirm local arrangements for this scheme.

The following DESs are rolled over for 2013/14 and practices can choose to continue to provide them.

  • extended hours
  • patient participation
  • alcohol reduction scheme
  • the learning disabilities health check scheme

Service specifications and audit requirements for new and amended DESs can be found here We are awaiting further guidance on the Patient Participation scheme and we will ask practices to participate in this scheme once the guidance has been published.

NHS England (London Region) is currently reviewing the Minor Surgery and Influenza and Pneumococcal DESs with a view to standardising pricing and monitoring arrangements for these schemes across the region from 1 July 2013. We will consult with London-wide LMCs on these new pricing arrangements. Existing schemes will be rolled over for a further 3 months from 1 April 2013. Please note that this only refers to schemes where the DES is commissioned from practices; where a LES scheme is offered CCGs will make arrangements with practices for provision of these services.

Immediate action requiredby practices

  1. Review the service specificationsand requirements for directed enhanced services listed in the Participation Agreement at Appendix One.
  2. Complete the attached Participation Agreement indicating the services your practice would like to provide in 2013/14.
  3. Submit the completed form y 31 May 2013[1].

NHS England will follow up practice returns as appropriate to ensure that the contract held with NHS England reflects the services commissioned from the practice.

Appendix One: Participation Agreement for Directed Enhanced Services to be commissioned from GP contractors in 2013/14

This document constitutes an agreement between NHS England (the commissioner) and a GMS/PMS or APMS contractor (the contractor) in respect of delivering the enhanced services. This document will become part of the contract documentation between NHS England and your practice to provide Directed Enhanced Services. Any requirements of your contract applicable to the provisions of these enhanced services including notice periods and dispute processes will apply.

By entering into this agreement the contractor enters into an arrangement to deliver enhanced primary medical services:

(i)in line with the requirements of the service specification published by NHS England which is deemed to be a part of this agreement and,

(ii)for the financial year 2013-14.

The contractor reserves the right to withdraw from the enhanced service by giving 3 months notice to the commissioner. The commissioner reserves the right to terminate this agreement should the contractors GMS/PMS/APMS contract be terminated or be subject to such conditions that in the reasonable opinion of the commissioner warrant early termination.

If practices areproviding Locally Enhanced Services or services within their APMS or PMS contract that are similar to any specification of a DES they will not be able to provide the DES in addition to those.   • That you understand post-payment audits may be undertaken by NHS England to verify achievement under any of the schemes
  • That you usethe read codes detailed in the DES specifications(where specified)in order to ensure accurate payments are made*.

*For the new and existing clinical enhanced services it is essential that practices use the read codes detailed in the DES specifications. Local read codes will not be picked up by the new General Practice Extraction Service (GPES), upon which payments will be based.
