PRESERVATION OF RECORDS OF HISTORIC SITES COMMITTEE ANNUAL REPORT

Listed below are suggestions for reporting historical sites and information needed on each. Each site reported must be related to the War Between the States or to the Confederacy.

ALL HISTORIC SITES: As per instructions, provide site name and address. If address is unknown or unavailable, provide street intersections. Include town, county, and state. If the site is not in town, provide mileage to nearest town. Do not write a description if a current photograph or brochure, providing a description, significance of the site, hours of operation, admission fee, and ownership, is attached. WRITE THE NAME OF THE SITE ON THE BACK OF PHOTOGRAPH AND USE A STAPLE TO FIRMLY ATTACH IT TO THIS FORM.

CONFEDERATE STATUES, MONUMENTS, OR MARKERS LOCATED AT CONFEDERATE SITES. Current photographs showing clear inscriptions may be attached. List organization placing the marker, date it was placed, and cost only if known. DO NOT SUBMIT INDIVIDUAL STATUES, MONUMENTS, OR MARKERS THAT ARE NOT LOCATED AT CONFEDERATE HISTORIC SITES (i.e. COURT HOUSE SQUARES). THIS INFORMATION SHOULD BE REPORTED TO THE MONUMENTS AND MARKERS COMMITTEE.

CONFEDERATE CEMETERY OR SECTION OF CEMETERY. Date established and number of Confederate graves is traditionally indicated on historic markers on site. Photographs of Confederate statues and memorial monuments showing inscriptions should be firmly stapled to the form. THIS INFORMATION SHOULD ALSO BE SENT TO THE MONUMENTS AND MARKERS COMMITTEE. DO NOT REPORT INFORMATION ON INDIVIDUAL GRAVESTONES OR HEADSTONES. THIS INFORMATION SHOULD BE SENT TO THE RECORDS OF INTERMENTS OF CONFEDERATE VETERANS (GRAVES AND CEMETERIES) COMMITTEE.

CONFEDERATE MUSEUM. Record name of museum on front of this form and firmly staple a brochure to the form. Information will be printed in the brochure.

HISTORIC HOME, CHURCH, OR OTHER BUILDING. If a brochure is not available firmly staple a photograph to this form. Original owner, date built, and name of present owner should be included on front of this form if known.

Chapter Chairmen should submit reports to the Division Chairman who compiles reports and submits to the General Chairman.
