Notes A School COSHH form in Word is available on the School Server.

Available from the Health and Safety Unit.

Guidance on making an assessment is given in Making a Chemical Hazard and Risk Assessment.

Guidance is also available from Guidance on Completing the Chemical Hazard and Risk Assessment Form.

Use a continuation sheet to expand any section of this form in hard copy version.

1 / LOCATION OF THE WORK ACTIVITY / N114
2 / PERSONS WHO MAY BE AT RISK
List names where possible / R.Perry,S.Thomas,M.Stanley,A.Humphries,K.Marshall,J.Finlay,A,Jennings,P.Patel,R. Nessa , project students and any other persons working in N114.
3 / ACTIVITY ASSESSED / Preparation of western Blot Substrate.
4 / MATERIALS INVOLVED / Attach copies of data sheet(s)
NAME / AMOUNT
max/stored / HAZARD / RISK PHRASES / HAZDAT NO*** / BIOSCIENCESNO***
Nitroblue tetrazolium salt (NBT) / 1g / Harmful / Harmful by inhalation, ingestion and by contact with the skin.
N,N-Dimethylformamide (DMF) / 250ml / Toxic / Toxic by inhalation and skin contact
May cause harm to the unborn child
Irritating to eyes, skin and respiratory system.
5 / INTENDED USE**
Give brief details and attach protocol/instructions / 0.5g of (NBT) is added to a 70% solution of (DMF) which is made up in water in a fume cupboard. Final solution volume is 10ml. Quantities
of between 100μl and 500μl maximum from this stock solution are then mixed with (BCIP) at similar volumes-see form number 1791 in a total volume of between 10 and 50ml and used to develop western blots.
6 / RISKS to HEALTH and SAFETY from INTENDED USE
From personal exposure or hazardous reactions. Refer to OELs, flash points, etc., as appropriate. Are pregnant women, breast-feeding mothers especially at risk?
Main risk is from accidental skin contact or inhalation.
DMF may cause harm to the unborn child.
Will cause irritation to eyes and skin
7 / CONCLUSIONS ABOUT RISKS
Is level of risk acceptable? Can risk be prevented or reduced by change of substance/procedure? Are control measures necessary?
Risk is minimal so long as good chemical practice is followed and the correct protective clothing is worn such as lab.coat, gloves and eye protection.
Amounts used in making up stock solution and in procedure are relatively small so hazards should be reduced.
8 / CONTROL MEASURES
Additional to Good Chemical Practice
Wear protection clothing as described in (7) above and work in a fume cupboard
9 / INSTRUCTION/TRAINING
Specify course(s) and/or special arrangements.
none
10 / MONITORING
Performance of control measures,
none
Personal exposure / Health Surveillance
11 / WASTE DISPOSAL PROCEDURE
See School Server for Approved Procedure Document on specific Chemical Waste Disposal.
For diluted solution run to waste down sink with plenty of running water down fume cupboard sink
For stock chemicals (DMF) dispose of in waste solvent bottle
12 / REVIEW
Enter the date or circumstances for review of assessment (maximum review interval 5 years)
5 years or sooner if quantities or procedures alter significantly
13 / EMERGENCY ACTION
TO CONTROL HAZARDS / To stabilize situation eg spread absorbant on liquid spill; eliminate sources of ignition, etc.
Spread absorbant on liquid spill and wipe down area thoroughly with water.
TO PROTECT PERSONNEL / Evacuation, protection for personnel involved in clean-up, Special First Aid
Ensure that the correct protective clothing is worn, gloves, goggles and lab.coat
Eyes: Irrigate thoroughly with water for at least 10 minutes. If symptoms persist obtain medical attention
Lungs: remove from exposure. If symptoms persist obtain medical attention
Skin: Wash thoroughly with water. In severe cases obtain medical attention
Mouth: Wash out mouth thoroughly with water. . In severe cases obtain medical attention.
Contaminated clothing should be washed before re-use.
TO RENDER SITE OF EMERGENCY SAFE / Clean-up/decontamination
Small spillage: Wash with plenty of water and detergent
Large spillage: spread absorbant material over contaminated area.
