Stronger Families Programme and Think Family Approach
Consent Form
In County Durham a new programme has been developed to support families. This is called the Stronger Families Programme. Alongside this, partner agencies are developing a new way of delivering services to families called the Think Family Approach.
This programme and the approach ensures that your family receive the right support at the right time.In order to make access to services and support easier, we have developed a way which brings services together and has one link person, known as a Lead Professional,that will be a single point of contact for you and the whole family. This will stop you having to contact a range of services as this one person will be your key contact. Your participation in this process will be required to ensure the support provided is being delivered in the appropriate way to best suit your family’s needs.
Due to the multi-agency approach, we will need to share your information with other services to ensure you all receive the best quality of care.
- I understand that the Stronger Families Programme and Think Family Approach is consent based and requires me to agree to the plan of work;
- I understand that as part of this work I will share information with the team of professionals who are working with me and my family,and that this will be used for the purpose of assessment and planning for me and my family. Agencies involved in this work could include health, council services, education, job centre plus, police, probation and others;
- I understand that the Lead Professional/Key Worker working with me may request information about me from other agencies who either work with me now or who have worked with me in the past. This information will be used as part of the assessment and will be shared with me;
- Where written information from other agencies is shared with the Lead Professional, I understand that the Lead Professional will share this with me first;
- I understand that the information I share with the Lead Professional will be stored securely and used for the purpose of providing services to me and my family;
- I have had the reasons for information sharing explained to me and I understand those reasons;
- I have had the opportunity to ask questions about the Stronger Families programme and Think Family Approach and about how my information will be used;
- I understand that if I do not consent to share information this could affect the services I receive;

- I agree to work with the ‘Team around the Family’ and for the information I provide to be shared with the agencies involved in supporting me. This in turn allows other services that I use to share information about me and my family for the purposes of providing me and my family with services and support;
- I understand that the safety and welfare of children and vulnerable adults is paramount and in some circumstances professionals can share information without my consent;
