Position Objective
The Day Program Case Manager & Group Facilitator is responsible for supporting the Therapeutic Day Program Coordinator (Coordinator) in the creation and maintenance of a safe therapeutic environment. The key purpose of the positionis to implement and review an evidence based program that assists participants to establish healthier relationships and lifestyles free of problematic alcohol and drug use.
Role Responsibilities:
Key responsibilities for this position include:
- Engaging with stakeholders to facilitate referrals to the program;
- Engaging suitable clients, including conducting complementary client assessments, to develop mutually agreed individual treatment plans (ITPs) and referrals;
- Facilitating the intake and assessment process in collaboration with internal and external service providers, including assessing the suitability of clients referred to the program;
- Facilitating program information group sessions and access to accommodation support for clients;
- Ensuring that clients on a waitlist have Interim Support Plans to avoid relapse;
- Working with clients to develop, implement and review their individual treatment goals including aftercare plans;
- Collaborating with existing service providers and facilitate referrals of clients to other services;
- Assisting in the establishment of inter-agency relationships including, if necessary, the development of formal protocols;
- Contributing to the development, implementation and delivery of group program modules ensuring materials and activities are planned in advance and the program is delivered in a timely and effective way;
- Collecting and recording client information and participating in treatment team reviews, case reviews and other relevant meetings ensuring information and client files are accurate and up to date;
- Supporting the work of the Coordinator through participating actively in team meetings and processes, regular reports on client progress, including identifying possible areas of concern as and when they arise;
- Assistingin the development and delivery of a manualised cognitive behavioural therapy program to clients inclusive (including individual monitoring sessions for a smaller caseload of clients);
- Contributing to the development and day to day operation of the program through participation in selected social/recreational activities and modelling appropriate behaviour;
- Facilitating a range of therapeutic and psycho-educational groups and activities, including groupinterventions for families and significant others using contemporary evidence based practice encompassing family sensitive, dual diagnosis and trauma informed care; and
- Participating in ‘housekeeping’ activities that support the day to day operation of the program.
Key Selection Criteria
- Comprehensive understanding of alcohol and other drug use, and an understanding of community based rehabilitation or day programs.
- Demonstrated experience delivering quality therapeutic outcomes for clients.
- Excellent group therapy and case management skills along with the ability to work with difficult and demanding clients.
- Ability to use best practice principals in all clinical work such as Family Inclusive Practice, Dual Diagnosis Principles and Trauma Informed Practice.
- Demonstrated proficiency in the application of Motivational Enhancement and Cognitive Behavioural Therapies.
- Ability to show initiative and capacity to work independently, adapting to meet changing organisational needs.
- Strong organisational and highly developed interpersonal and communication skills (both verbal and written) with a consultative approach to addressing issues.
- Ability and willingness to contribute and work effectively in a team environment.
- A positive, friendly and optimistic attitude.
Essential Requirements
- Satisfactory outcome of a confidential Police Check and Working with Children Check.
- Certificate IV Alcohol & Other Drugs, or willingness to complete.
- Empathy for those whose lives have been affected by problematic alcohol and other drug use/ and or mental health disorders.
- Possession of a current Victorian Driver’s License.
- Information technology skills, including proficiency in Microsoft Office suite.
- Eligibility to work in Australia.
Desirable Requirements
- Relevant Health, Social or Welfare related qualification and experience.
- Specialist knowledge in one or more relevant areas such as Family Therapy, Mental Health, Family Violence, Trauma, etc.
- First Aid Certificate.
- Current knowledge of OH&S practices.
- Experience working within a Day Program or Therapeutic Community model.
