PURPOSE OF JOBTo provide an initial assessment and interviewing service for older people services and an assessment and care management service to older people with non-complex needs and their carers in accordance with Council policies.

SPECIFIC DUTIES AND RESPONSIBILITIES

  • TO UNDERTAKE CONTACT AND OVERVIEW ASSESSMENTS.
  • TO IDENTIFY RISKS FROM PRESENTING NEEDS.
  • TO DETERMINE ELIGIBILITY FOR SERVICE PROVISION.
  • TO FORMULATE, IMPLEMENT, MONITOR AND REVIEW PACKAGES OF CARE ARRANGED TO SUPPORT OLDER PEOPLE LIVING IN THEIR OWN HOMES.
  • TO REVIEW THE NEEDS OF OLDER PEOPLE LIVING IN LONG STAY RESIDENTIAL HOMES.
  • TO ARRANGE SUPPORT SERVICES FOLLOWING AN EMERGENCY ASSESSMENT OF ELIGIBLE NEEDS.
  • TO IDENTIFY RISK OR INCIDENCE OF ABUSE AND INSTIGATE ADULT PROTECTION PROCEDURES AS REQUIRED.
  • TO CO-WORK WITH SOCIAL WORKERS AND QUALIFIED ASSESSORS IN CARE MANAGING MORE COMPLEX CASES.
  • TO IDENTIFY CASES WHICH MAY REQUIRE THE INVOLVEMENT OF A QUALIFIED SOCIAL WORKER OR QUALIFIED ASSESSOR.
  • TO WORK UN-SOCIAL HOURS AS REQUIRED BY THE DEMANDS OF THE WORK.
