6. / BRIEF RESUME OF THE INTENDED WORK
6.1 / NEED FOR STUDY:
One of the misconceptions that we hear from our patients that “it takes long time for homoeopathy to work”. It is alright in chronic conditions, but in acute conditions people preferred to go to allopath and get it treated. After the allopath treats the acute episode patient brought back to homoeopath for total cure by raising the immunity, and thus homoeopathic treatment continues to survive with the common belief that homoeopathy is safe and good for raising immunity and total cure.
Hahnemann wanted to speed up the cure as much as possible within that time frame. He experimented throughout his life to improve the homoeopathic system of medicine for the benefit of the patient. He felt that his great discovery was LM potency.
To evaluate the efficacy of LM potencies in homoeopathic general practice is one of the major focuses of this study.
6.2 / REVIEW OF LITERATURE:
The 50 millesimal scale or LM potency was introduced by Dr. Hahnemann in 6th edition of Organon of medicine in aphorism 2701.
Potencies prepared under this method are named by Dr. Pierre Schmidt as “50 millesimal potencies”, as the material part of the medicine is said to be decreased by 50,000 times for each degree of dynamisations1.
In India and Bangladesh, the potency prepared by this scale is designated as 0/1, 0/2, 0/3 and m/1, m/2, m/3 etc. In west it is designated as 1/0, 2/0, 3/0 etc. The numerator “0” representing symbolically the poppy seed sized globule employed in each dynamizations2.
To produce LM1 potency, dissolve 60 mg of 3c trituration of the substance to be potentised in 20 ml of ethanol 15 percent. Transfer 1 drop of the solution to a small vial, add 2.5 ml of ethanol 86% and shake vigorously 100 times. Moisten 100g of size 1 granules evenly with solution, following impregnation in a closed container the granules are air dried they represent LM1 potency2.
DR. R.P. Patel: I have tried Hahnemann’s way of administration as well as my own ways and I am always successful. The simplest way according my observations, I have found are:
  1. Prepare one dose in 2 grains of sugar of milk with 2 pills (No 10) of selected potency of medicine for chronic disease.
  2. Dissolve 2 pills or even 1 pill (No 10) of selected potency of selected medicine in one ounce of water and ask the patient to take a teaspoonful or 10 drops or one dram in ½ ounce of water every ½ or ½ to 1 hour after stirring according to severity of the disease. In chronic cases once a day.
  3. Dissolve one dose (2 pills) of selected potency of indicated medicine in one ounce of water and ask the patient to take ½ ounce in the morning and half ounce in the evening after stirring, in moderate type of cases where there is no immediate danger3.
Dr. Hahnemann:For the same reason the dose of homoeopathic medicine increases the quantity of fluid in which it is dissolved when administered to the patient, although the actual amount of medicine remains the same. For in this case when the medicine is taken it comes to a much larger surface of the sensitive nerves responsive to medicinal action. Although theorists may imagine there should be a weakening of action of medicine by dilution with large quantity of liquid, experience asserts exactly the opposite. At all events when medicines are employed homoeopathically4.
Dr. Hahnemann: But the chronic diseases, on the other hand, a single dose of appropriately selected homoeopathic remedy will at times complete even with but slowly progressive improvement and give the help with such a remedy in such a case can accomplish naturally within 40,50,60,100 days. This is however but rarely the case; besides it must be a great matter of importance to the physician as well as patient that were it possible, this period is diminished to one half, one quarter, or even still less, so that more rapid cure might be obtained. And this may be very happily affected, as recent and often repeated observation have thought me under following conditions; firstly medicine selected is at most perfectly Homoeopathic, secondly, if it is a highly potentised it is dissolved in water and given in a small dose as experience as the most suitable and definite intervals for the quickest accomplishment of cure but with precaution, that the degree of every dose deviate somewhat from the preceeding and following5.
“ what I said in the 5th edition of Organon, in long foot not of this paragraph in order to prevent the undesirable reaction of the vital energy of all that experience I then had justified? But during last four or five years, however, all the difficulties are wholly solved by my new altered but perfected method5.
Dr.Hahnemann: since I have last addressed the public (beginning of 1834) concerning our healing art, I have had other things also the opportunity to gain the best possible mode of administering the doses of the medicine to the patient, and I herewith communicate what I have found best in this respect 6.
Dr. Hahnemann: “experience has shown me as it has no doubt as shown to most of my followers, that it is most useful in disease of any magnitude. To give to the patient the powerful homoeopathic pellet or pellets in solution, and this solution in divided doses, in this way we give the medicine dissolved in seven to twenty table spoon without any addition, in acute and very acute disease every six, four or two hours; where danger is urgent, every hour, every half hour tablespoon at that time, with weak persons and children’s only a smart part of table spoonful ( one or two tablespoons..) may be given as a dose. In chronic cases it is best to give the dose (eg: spoonful) of a suitable medicine atleast every two days, more usually every day6.
Dr.C.M.F. von Boeninghousen: In new edition of Organon improved and completed by Hahnemann himself the new simplified method for potentising the medicines will be thought, which has considerable advantage over the former and yields the preparation as to the efficiency of which I can, from own experience give full praise7.
Dr. Charles pahud: “with these marvellous dilutions one can obtain marvellous cure too and shorter time than with usual centesimal scale potency”8.
The trend is clear: Hahnemann has evolved homoeopathy into a direction which requires more and more individualisation. Apart from the right remedy (and potency), the homoeopath is now required to choose the size of the dose on an individual basis. The 'split dose' in which a small part of a single globule can be administered in form of an aqueous solution permits this. In addition, this mode of administration seems to let the remedies take hold at a deeper level than possible before9.
Schmidt.P: For at least the last years of his life Hahnemann used both his centesimal and LM potencies exclusively in the medicinal solution with the addition of a dilution glass. In 1843, just prior to the Master's death, Hahnemann sent Boenninghausen two cases in which he used the C and LM potencies in exactly the same fashion. This proves two important points; first that the new methods of using the aqueous solutions is for all homeopathic remedies. Hahnemann considered the two potency systems to be complementary and used them both to increase the range of the homeopathic pharmacy. Now he had 6c to 1M centesimal and 0/1 to 0/30 LM potencies at his disposal10.
Dr.Rajeev saxena: Upto 5th edition, the Organon was nearing complete but the 6th edition of Organon was ‘most nearly perfect to all’. Hahnemann was well aware of the limitations of centesimal scale that is why in sixth edition he opined that this new method (LM potency scale) was perfect method in footnote aphorism 246 he opines this fact11.

"In the new edition of the Organon which will probably appear yet in the course of this year, improved and completed by Hahnemann himself, a new simplified procedure for the potentizing of medicines will be taught, which has considerable advantages over the former and yields a preparation as to the efficacy of which I can, from my own experience, give full praise. I know this procedure but according to my pledged word of honor, am not as yet permitted to communicate it to anyone”.13
Around the year 1840 the Founder began to introduce his new LM potency into clinical practice to complement his C potencies. The discovery of the new 1/50, 000 dilution ratio introduced a new potency system with unique medicinal qualities14.
Dr. Harimohan Choudhry: The Organon has recommended potencies from LM/1 to LM/30 only. Vitality of the human body is limited. So Hahnemann has put a limit to the medicine upto 30 for the treatment of sick human beings.15
6.3 / AIMS AND OBJECTIES OF THE STUDY:
To evaluate the scope and limitations of LM scale
potencies in homoeopathic general practice, both in acute and chronic diseases.
Objectives:
The study aims to study the –
History of LM.potency.
Preparation of LM.Potencines.
To access the utility of the LM.Potencines in day to day practice, in various disease conditions.
Prescribing methods of LM. potencies.
7. / MATERIALS AND METHODS:
7.1 / PRIMARY SOURCE:
The subject for this study will be collected from OPD/IPD/Rural camp of Dr. B.D. Jatti Homoeopathic Medical College, Hospital and Post Graduate Research Centre, Dharwad.
7.2 / METHOD OF COLLECTION OF DATA
Simple random sampling technique.
To compare the effectiveness of two types of potencies, the fifty millesimal and centesimal potency, the patients were divided in to two groups.
Group I: It consisted of cases treated with fifty millisimal potency. All cases were taken in standardized case record.
Group II: This consists of cases treated with centesimal potency. All cases were taken in standardized case record.
The name, age, sex, address and detailed history of each patient were recorded.
INCLUSION CRITERIA:
Patients of age group ranging from 20 to 45 yrs of all socio economic status will be considered. Patients will be selected on the basis of inclusion criteria, history and clinical findings. Detailed case history as per Proforma will be taken.
EXCLUSION CRITERIA:
1.Cases with complication and pathological changes.
2.Old age people and paediatric cases.
STUDY SAMPLING DESIGN:
As the university has prescribed minimum 30 cases in the two years time bound study, 30 cases taken in control group for prescribing centesimal scale potency, and 30 patients taken for experimental group study for prescribing 50 millesimal scale potency.
Sample size: Not required hence as specific disease is not considered.
Study period: from Nov 2009 to Nov 2011
STUDY DESIGN:
The group of patients selected were from both sexes of adult age group. The diagnosis was based on clinical presentation and the sampling techniques and purposive sampling .Diagnostic criteria are mainly based on clinical presentations, as well as lab investigation.
FOLLOW UP:
The follow up of the cases depends on the severity of clinical features, preferably once in 15 days.
