GENERAL SUMMARY:

Under the direction of the Director of Nursing, responsible for overseeing, coordinating, and ensuring accuracy and completeness of the MDS process so that submission is timely and reflects state and federal regulations to meet CMS survey process and PPS reviews.

ESSENTIAL JOB FUNCTIONS:*

  • Transmits assessments to state and federal agencies ensuring no late assessments.
  • Assures documentation compliance by monitoring documentation in the residents’ health records and coordinating MDS time reference period with the nursing and rehab services; reviews daily nurses’ notes, physicians’ orders and progress notes and care plans as appropriate; monitors and assures the documentation for assistance with activities of daily living to ensure they reflect the residents’ care for the effective date of the MDS.
  • Educates staff of the need for supporting accurate and timely MDS documentation.
  • Reviews CAA (Care Area Assessment), care plans, and preliminary assessments for completeness and accuracy; addresses and follows up on all areas of concern.
  • Reviews facility’s CASPER on a monthly basis. Investigates and develops a plan to address any areas that flag. Reviews the report with the Administrator and DON on a monthly basis and implements staff educational plan to address areas of concern.
  • Coordinates the plan of care schedule to meet CMS requirements.
  • Leads Plan of Care meetings and ensures follow-up on questions raised by resident or family members.
  • Assists Nursing staff in ensuring care plans are updated and accurate at all times.
  • Develops PPS assessment schedule for new admissions and coordinates same with the Rehabilitation Department.
  • Provides in-service training of the MDS process to all staff, as needed.
  • Serves as Chairperson of the Utilization Review Committee.
  • Participates in the monthly triple-check meetings.
  • Collaborates with the Finance Office as needed to address discrepencies.

ESSENTIAL JOB FUNCTIONS (continued…):*

  • Participates in the pre-admission screening process and requests additional information on prospective admissions as needed.
  • Performs timely certification / recertification for physicians for skilled residents.
  • Communicates to inter-disciplinary team all significant changes.
  • Remains informed on any resident change of status, including transfers and discharges.
  • Stays abreast of CMS/OBRA/PPS requirements.

OTHER DUTIES AND RESPONSIBILITIES:

  • Promotes and maintains positive community relations.
  • Maintains current knowledge of trends and practices in the field through peer association, attendance at seminars, study, and review of literature.
  • Responsible for maintaining required number of CEU’s for re-licensure.
  • Submits to a TB skin test as required, unless exempted by the Employee Health Nurse.
  • Reports any workplace safety concerns to Supervisor immediately.
  • Attends all mandatory in-services.
  • Overtime may be mandated, as necessary, at the discretion of management.
  • Assumes on-call duty on a rotating basis in the on-call pool.
  • Performs other related duties as required.
  • Maintains the confidentiality of all resident and facility information, and ensures such information is not released without proper authorization.

SKILLS/EXPERIENCE/TRAINING REQUIRED:

Duties require knowledge of MDS, completion of a Bachelor's Degree and five to seven years of related experience, or equivalent combination of education and experience.

Demonstrated experience regarding the interpretation of quality measure data and the development of systems to address and resolve flagging areas on the CASPER report.

Must be licensed as a Registered Nurse in the State of New Hampshire at time of hire.

SKILLS/EXPERIENCE/TRAINING REQUIRED (continued):

Must have knowledge of and comply with relevant State and Federal laws, rules and regulations, including Administrative Rules of the N.H. Board of Nursing.

Knowledge of long term care and geriatric nursing.

Ability to demonstrate good judgment in dealing with residents and their families.

Ability to demonstrate ethical behavior regarding confidentiality and residents’ rights.

Must be certified in cardiopulmonary resuscitation.

Must possess computer literacy and knowledge of MDS process and PPS requirements.

Ability to coordinate in-service education for the nursing staff.

Displays good clinical judgment.

Organizational skills with an eye for accuracy and attention to detail.

Ability to work under pressure to meet deadlines.

Ability to obtain cooperation and desired outcomes from nursing staff.

Ability to communicate effectively both verbally and in writing; to establish positive public relations for the County; and to interact effectively with a wide variety of people.

SUPERVISORY RESPONSIBILITY:

This position is responsible for supervising the activity of Nursing Home staff as it relates to the MDS process.

WORKING CONDITIONS/PHYSICAL DEMANDS:

Work is performed primarily in a nursing home environment, which is generally not subject to extremes in temperature, odors, etc.

May use computer keyboards requiring eye/hand coordination and finger dexterity.

More than half of time spent in a normal office setting.
