6. BRIEF RESUME OF THE INTENDED WORK.

INTRODUCTION:

6.1 NEED FOR THE STUDY:

In Modern era, the advancement in science and technology has influenced mankind very much. Due to this he became in need to change his lifestyle, leading to his more working time compared to rest. Generalized hyper mobility associated with occupation that involves heavy weight lifting, twisting, carrying heavy loads on head and shoulder, prolonged driving, trauma etc contributes to increase of Vatadosha as well as manifestation of Vatavyadhis like Avabahuka.

In Modern science also several shoulder disorders are there like Frozen shoulder, Rotator Cuff Syndrome ,Shoulder Hand Syndrome, Deltoid Bursitis, etc and the treatment given was NSAIDS, Physiotherapy etc.There is no specific medication and the medications may not show satisfactory effect and may produce side effects therefore, there is a need to assess the effect of Ayurvedic medication in the management of Avabahuka.

Nasya and Pana is the prime modality of treatment in Urdhwajatrugatarogas and Avabahuka is one of those vikaras.

Dashamoola, Bala Masha Kwatha, Taila and Ghrita Nasyachikitsa has been explained in the management of Avabahuka in Chakradutta Vatavyadhi Chikitsa & in YogratnakaraVatavyadhi Chikitsa. Andin the context of Bahusheershagatavata as Uttarabhaktikasnehapana has been mentioned in CharakaChikitsasthana. And also mentioned in AstangaHridaya Vata Vyadhichikitsa sthana the line of treatment for Avabahuka is Uttarabhaktikasnehapana .

As Nasya and Pana both have been mentioned in Avabahuka chikitsa,here we want to know the which one of the procedure gives better result either orally or nasal route with specific medication called DashamuladiYamaka Sneha.

Dashamuladi Yamaka Sneha has shoolahara, shothaharaetcproperties; therefore there is a need to evaluate whether the Dashamuladi Yamaka Sneha gives better result in the form of Uttarabhaktika Nasya or Uttarabhaktika Snehapana. Hence study is planned to compare the effect of Nasya and Pana with DashamuladiYamakaSneha in the management of Avabahuka.

6.2 REVIEW OF LITERATURE:

Avabahuka: It is a condition in which vitiated vata associated with kapha lodges in Amsapradesha and by contractions of siras leads to pain and stiffness in the joint and muscles of shoulder and upper arm and causes spandana of bahu leading to avabahuka.1

Nasya and Snehapana has mentioned as a line of treatment in Vatavyadh 2

Dashamuladi Yamaka Sneha has been specially mentioned for the treatment of Avabahuk.3 It comprises of Brihatpancamula, Laghupanchamula, Masha, Bala, Tilataila and Ghrita, Its action is Vatakaphahara, Shothahara, Shulahara, Balya and Brihmana.Dashamuladhi Yamaka Sneha is very much effective in pacifying the Vataroga and it can be used as Nasya and Pana.

6.3 OBJECTIVES OF THE STUDY:

•To study Avabahuka in detail.

•To study the efficacy of Dashamuladi Yamaka Sneha as UttarabhaktikaNasya in the management of Avabahuka.

•To study the efficacy of ofDashamuladi YamakaSneha as UttarabhaktikaPana in the management of Avabahuka.

•Comparative study of Uttarabhaktika Nasya and Uttarabhaktika Pana in the management of Avabahuka.

7. MATERIALS AND METHODS:

7.1 SOURCES OF DATA

1. Patients will be selected from Sri Siddharoodha charitable hospital of N.K.J.Ayurvedic medical College and P.G.researchcentre.Bidar.Karnataka.

2. Ayurvedic classics, National and International journals, magazines, seminars, conferences, digital library and web sites.

7.2.METHOD OF COLLECTION OF DATA:

1. Patients will be selected by simple randomized sampling procedure.

2. Separate case sheet will be prepared, mentioning the signs and symptoms of Avabahuka and treatment chart of Dashamuladi Yamaka Sneha Nasya and Pana. Based on the same follow up study is carried out.

3. Procedure will be conducted under the supervision of Guide, Co Guide and expert departmental staff.

RESEARCH DESIGN

•30 patients will be selected irrespective of sex, occupation, socio-economic status with all signs and symptoms of Avabahuka by simple randomized sampling method. These 30 patients are divided into 2 groups.

•Group A - Patients will be treated with Dashamuladi Yamaka SnehaNasya for 7 days, 16 drops (8 drops in each nostril) daily Sayankala after meals.

•Group B - Patients will be treated with Dashamuladi YamakaSnehaPana for

7 days 30 ml in divided doses.

•Before starting treatment thorough counseling of patients & brief explanation regarding the procedure of Nasya karma and Pana will be done. All the patients should stay in specific diet chart during the therapy.

SELECTION OF CHIKITSA

•Dashamuladi Yamaka Sneha in the form of Uttarabhaktika Nasya and Pana.

SCHEDULE OF NASYA and PANA

•Nasya-for 7 days once in a day after meals.

•Pana-for 7 days twice in a day after meals.

METHODOLODY:-

The progress will be noted on the basis of assessment parameters (both subjective and objective), before treatment, & after treatment in a specially prepared case sheet.

STATISTICAL ANALYSIS:

The observation will be analyzed Scientifically by employing some suitable statistical test between the group and within the groups.

SELECTION CRITERIA

INCLUSION CRITERIA:-

1.Classically diagnosed cases of Avabahuka.

2.Patients irrespective of sex.

3.Patients within the age group of 30-60 years.

4.Patients fit for Nasya karma and Snehapana.

EXCLUSION CRITERIA:-

1.Patients below 30 years and above 60 years of age.

2.Anarha for Nasya and Snehapana according to classics.

3.Uncontrolled DM & systemic disorders other than that of Avabahuka

4.Patients of shoulder joint dislocation / fracture.
