Position Title: Health Home Program Supervisor
Position Summary: The Health Home Program Supervisor oversees daily service operation of Full Life Care’s Health Home person-centered and community-based care coordination program. This includes but not limited totraining and onboarding of program service staff including care coordinators and allied staff, day-to-day clinical supervision, as needed case problem-solving and crisis response, caseload monitoring and tracking for state-contracted program requirements and internal policies and procedures. Program Supervisor is responsible for day-to-day operations of Health Home care coordination program and oversees care coordinator staff maintaining caseloads of 50-55 clients as well as allied staff activities.Program Supervisor is also responsible for maintain state-certified Health Home Trainer credentials and will provide two-day trainings as needed per state requirements. The Program Supervisor assures integrity and compliance of services provided throughout King County.

Essential Position Functions:
- Responsible for direct supervision of Full Life Care Health Home care coordinator(s) and allied staff. Participates in recruitment and selection of care coordination and allied staff. Orients and trains care coordinator and support staff.
- Manages corrective action procedures and disciplinary actions for program care coordinator and allied staff.
- Oversees timekeeping and payroll functions in coordination with Full Life Care financial department.
- Works closely with Health Home Program Director on program growth and development.
- Develops and implements staffing patterns that promote appropriate utilization of staff.
- Promotes effective Health Home services in compliance with agency philosophy, policy and procedures, and in accordance with state contractual guidelines.
- Maintains internal program service tracking according to internal expectations as well as state-wide success metrics.
- Oversees outreach efforts and manages tracking of due diligence efforts to engage referred clients in services.
- Provides ongoing clinical supervision to program staff. Maintains and records ongoing clinical review of staff documentation and Health Action Plan client assessments; provides feedback to staff and monitors follow-up.
- Oversees monthly internal program billing process and responsible for monthly billing report submission to Health Home Director and Quality Assurance Specialist.
- Oversees grievance process for program beneficiaries.
- Assist in preparation for, and participate in, periodic internal audits and state audits by Health Care Authority and the Department of Social and Health Services of program.
- Maintains state-certified Health Home Trainer credentials and provide Lead Health Home-required 2-day trainings.
- Establishesand fosterscommunity partnerships to strengthen and support service network for program beneficiaries.
- Provides program education throughout community to agency partners and providers.
- Tracks and monitors fulfilled training requirements for program service staff.
- Assures training needs are available to and met by internal staff including scheduling, training, continuing education, skill development, and performance evaluation.
- Collaborateswith program staff to revise, update, and improve training on Health Action Plan and other care coordination issues.
- Actively promotes all agency programs with other community agencies.
- Performs other duties as required.
Marginal Functions of the Position:
- May serve as back up for Health Home program staff;
- Perform other duties as recognized or assigned;
- Attend various trainings and staff meetings as needed;
- Collaborate with and train staff on proper operation and maintenance of program systems and equipment;
- Availability to be reached and respond quickly to program and office facility emergencies including direct client intervention;
Minimum Qualifications:
- Master’s Degree in Social Work and three (3) years’ experience in social service program supervisory or management role, preferably in human service field and focused in community-based service provision;
- Established experience with Long Term Care system and Medicaid-funded programs;
- Knowledge of health needs and services required to assist elderly and disabled adults;
- Ability to read and write in English, ability to communicate verbally and in writing with other clinical and community providers, community partners, beneficiaries, families and supports, and contractors.
- Ability to supervise, motivate, direct, and train others;
- Ability to represent agency and program in community outreach and education efforts;
- Ability to plan, organize, prioritize and coordinate work assignments and/or projects;
- Ability to perceive and adapt to program needs, take initiative, and work independently with professional clinical judgment and minimum supervision;
- Ability to formulate organization and tracking systems;
- Ability to develop program policies and procedures for new service approaches and/or new areas of responsibility;
- Ability to formulate solutions to problems in a timely manner, prioritizing deadlines and triaging issues as needed;
- Ability to establish and maintain effective working relationships with agency and program network staff as well as general public/community partners;
- Ability to maintain professional boundaries and practice self-care;
- Ability to work and make clinically-informed decisions with efficiency under frequent pressure of deadlines while maintaining a professional demeanor;
- Ability to formulate solutions in timely manner;
- Sensitivity to the needs and values that are present in working with diverse groups of people;
- Demonstrated public presentation skills;
- Must have excellent verbal and written communication skills;
- Must have access to private reliable means of transportation; may require reliable vehicle for client transport;
- Current Washington State Driver’s License and proof of safe driving record obtained through appropriate Washington state department;
- Ability to effectively use computer and communication technology; knowledge and proficiency in Windows, Excel, Work, and Outlook.
Preferred Qualifications
- Licensed Independent Clinical Social Worker (LICSW) with three (3) years’ experience in social service program supervisory or management role, preferably in human service field focused on services in a community setting.

- Organized, self-motivated, honest, and mature;
- Must be able to lift 25lbs and support clients mobility;
- Must be able to operate office equipment including but not limited to computer keyboards, multi-line phone system, fax, and copy machines.
