POSITION: Disability Support Worker

CLASSIFICATION: Permanent/Part Time/Casual

REPORTS TO: Team Leader

Function/Purpose:

Respect and promote the dignity, independence and rights of each client.

Respect and maintain confidentiality relating to clients, house, staff and the service.

Ensure a high quality of service is delivered on an individual and service level

Specific Responsibilities/Accountabilities:

·  Be responsible for clients under your supervision at all times.

·  Respect and maintain confidentiality relating to clients, house, staff and Service.

·  Participate in the planning and implementation of client’s Individual Lifestyle Plans (ILP) as described in the ‘Anowah Individual Lifestyle Plan Process and Procedures Manual’.

·  Assist clients in developing all areas of their life, by implementing programs and using techniques specified in the client’s ILP.

·  Participate in the planning and implementation of specific individual’s positive intervention strategies as specified in the client’s ILP or where deemed necessary.

·  Collect relevant data in line with the ILP process.

·  Encourage and promote client’s participation in decision making in activities of everyday life.

Professional Support and Care / Support daily living and
individual needs (i.e. health, personal care, physical, emotional, spiritual, sexual, cultural, recreational, vocational and financial) of persons with a disability in order to promote maximum independence and participation at home and in the community.
Provide support in line with Anowah’s policy and procedures, organisational values, NSW Disability Service Standards and relevant Workplace health & safety requirements to ensure quality service outcomes to persons with a disability. akin / Data to support care being provided
IP data collection
Daily summaries completed for personal care & oral hygiene.
Nutritional records kept when necessary
Communication / Actively engage with parents, advocates and guardians in order to encourage positive and open communication
Encourage persons with a disability in decision making and support them to meet their own needs through appropriate communication to promote independence. / Completion of home diaries phone contact and meetings with significant others in clients life as evidenced by maintenance of personal contact documentation.
Team Contribution / Use a team based approach to ensure consistency and quality to service delivery to persons with a disability and the operations of the service outlet.
Assist the Team Leader in the orientation of new and casual staff on the needs, plans and programs of persons with a disability, to facilitate staff providing appropriate care and support.
Contribute to the environment of ongoing learning and development to ensure best practice standards / Attendance and contribution to team meetings and consistency of approach within the service outlet.
Appropriate communication to other members of the team via the communication book.
Record Management / Collect and maintain accurate, complete and current information on clients, workplace risks and incidents / All appropriate documentation is completed within the allocated time frames
House Management / Undertake the daily or routine tasks of meal preparation, cleaning, washing, basic
property maintenance and managing finance in order to support the efficient functioning of the household whilst actively engaging clients as much as possible.
Purchase according to planned budget and be accountable for finances and assets of the service outlet, including petty cash,
order to operate within allocated budget / Completion of all household routines evidence of same completed in the house diary.
Finances / Organising cash payments for internal accounting fees
Banking preparation with clients
Maintain pre and post banking documentation up to date. / Keeping client’s payments up to date
Regularly attending banking with clients
Learning & development / Identify and actively participate in ongoing learning and development to employ best practice standards in providing support and care to clients. / Attendance at structured
training course or informal programs.
Conduct / Ensure all activities comply with agency and government policies and guidelines,
including employment equity and diversity, WH & S, ethical practice and the Code of Conduct and Ethics to ensure
equitable and inclusive practices. / Follow ASSID code of conduct.
WH &S / Ensure systems are utilised to identify and address Workplace Health and Safety risks in the workplace in order to maintain a healthy and safe working environment that complies with Anowah policies and legislation. / Appropriate reporting of ALL possible WH &S issues via the correct procedure
