Position:Community Health Worker

Purpose:

To deliver a programme of community health work in relation to reducing chronic disease, reducing smoking, encouraging healthier eating and higher levels of physical activity. The health worker with work with all age groups in the community and in health settings to support people to adopt a healthier lifestyle. Enable local residents to access the services and activities they need to empower them and increase their confidence and well being.

MAIN DUTIES/RESPONSIBILITIES

The health worker will have five main roles to play;

  1. To work to the agreed objectives within the Enhanced Public Health Programme (EPHP) Business Plan.
  2. To increase active community participation in health promotion activities related to chronic diseases.
  3. Help to overcome behavioural and cultural barriersthat preventpeople from adopting a healthier way of life.
  4. Providing health information and connecting people to resources that exist in the community.
  5. Supporting local health professionals in the statutory sector, and those in voluntary and community groups.

KEY RESULT AREAS

  • To develop an understanding of current needs and activities in the locality around chronic disease.
  • To develop strong links with local partners and organisations in the NorfolkPark and Arbourthorne area.
  • Enable local residents to attend Public Health Interventions in the locality
  • To develop strong links to local health professionals, social services and voluntary sector.
  • To identify any barriers that might prevent someone accessing current services and to inform improvements.
  • To promote and publicise activities in a way that is audience appropriate.
  • To deliver the EPHP Business Plan to agreed deadlines and monitor progress as agreed.
  • To develop linkages with equivalent projects as agreed with line manager for peer support purposes.

  • To attend all required training and professional development (including Stop Smoking Advisor, Active Friends).
  • To be aware of NHS Sheffield policies and procedures and ensure that they are followed.

General Statement

  • In addition to these functions, employees are required to carry out such duties as may be reasonably required.
