Please complete this Questionnaire to provide information needed by the HRM Division for assessing the relative value of your position to the Company. Should additional space be needed for answering a question, please continue on the reserve side of the page. Your immediate supervisor will review your answers; if clarifications or revisions are needed, he/she will review these requirements with you prior to forwarding the completed Questionnaire to the Compensation Manager.

- Describe a typical project. How complex is it? What must you do? What is needed in terms of creativity or originality to achieve desired results?

- Describe the manner in which you receive assignments. For example, do you receive detailed step-by-step instructions, or are you told what is wanted and given general procedures to be followed? Provide an example.

- Do you receive well-defined instructions, or must you frequently use guidelines to make interpretations? Give examples.

- Characterize the general nature of your person-to-person contacts. Are they primarily to give information, to obtain information, to explain policies and procedures, to persuade others concerning the benefits of a program, to settle problems, etc.?
- Who must you contact for what specific purpose? Provide position titles and information on the affiliations of these individuals (other employees within the Company, customers, vendors, representatives of other companies, governmental representatives, etc.).
